JobSummaryUnder the direction of a Registered Nurse, LicensedPhysical, Occupational or Speech Therapist, assumes responsibilityfor the collection of data related to patient status, performanceof basic nursing procedures and therapeutic exercises, on-goingcollection and documentation of patient data; assistance withtreatments and procedures and assistance with maintenance ofpatient care equipment.
Accountabilities Provides age-appropriate patient care based upon individualizedplan of care as directed by the Registered Nurse (or LicensedProfessional) including the patient’s physical, emotional needs,cultural, spiritual, psychosocial and educational needs. Demonstrates effective use of time and resources in thedelivery of assigned patient care. Adheres to policies related to patient rights, confidentialityand customer service standards. Demonstrates effective communication skills in collaborationwith other health team members to ensure optimal functioning of theAgency. Performs basic nursing procedures and other specific tasks andtreatments including thermal application, non-sterile soaks,application of simple dressings, perineal care, foot care andtherapeutic exercise. Documents and reports patient status and condition which mayinclude vital signs, intakes and out puts, etc. to appropriate
RN,PT, OT, ST.
Adheres to established department policies, procedures, safety,environmental and infection control standards. Practices withinguidelines of all applicable regulatory agencies. Identifies opportunities for process improvement andparticipates in on-going process improvement activities. Maintains and / or ensures adequate maintenance of patientmedical records and treats as confidential. Maintains skills, agency-based competencies and completesannual mandatory continuing education requirements forcertification. Provides comfort and pain management measures Completes required Agency documentation accurately to includedocumentation of care, charge tickets and mileage reimbursement.
Supervisory/Management ResponsibilitiesThis is a non-management jobthat report to a supervisor, manager, director or executive.
MinimumRequirementsCompletion of a nursing assistant or patient care techprogram.
Required Certifications, Registrations, Licenses
CNA -NURSING
ASSISTANTValid driver’s license, an acceptable five-yearmotor vehicle record as defined by the Acceptable Motor VehicleRecord (MVR) Chart, and a certificate of auto insurance.
In Lieu OfThe Minimum Requirements Noted AboveIn lieu of school verificationof a nursing assistant training course, will accept current orformer South Carolina CNA certification as proof of completed staterequired training at time of hire.
