PERSONAL CARE ASSISTANT

(PCA)

Part Time, Evenings Job Summary:

Assists residents with bathing, dressing, personal hygiene, andADL’s as needed during shift. Changes linen on beds with bath schedule and/or PRN. Takes and records vital signs per policy and/or asdirected. Assist CNA’s with needed tasks. Turns residents while in bed per Facility policy. Repositions residents while out of bed per Facilitypolicy. Assists with oral hygiene including denture care when gettingup in the morning, after meals, at bedtime, and/or whenneeded. Answers call bells promptly following Facility policy. Completes patient care records at end of shift, includingaccurate I and O. At the end of month, fill in Patient Observation Records andActivity Attendance Records for the next month. Collects urine or stool specimens as directed. Gives enemas as directed. Keeps patient unit/halls clean and neat. Keeps bedside tables, chest of drawers, and closets clean, neatand organized. (Including clothing) Cares for and cleans nursing equipment. (BP equipment,thermometers, etc.) Follows restraint policy, including release. Assists with post-mortem care when needed. Assures residents have fresh ice water by refilling pitcherswith ice and/or water at least once every shift. Cleans water pitchers, cups, bedpans, urinals perschedules. Rinses personal clothing, sheets, etc. if soiled before placingin appropriate laundry barrel. Assists residents with bedpan or urinal. Empties catheter bags at end of shift and records output onappropriate record. Assists with rehabilitation programs such as bowel and bladderretraining, grooming, etc. and always charting on Rehabilitationsheet every shift. Assists charge nurse with resident needs and treatments asdirected. Keeps a clean towel and a clean washcloth for each patient inbathrooms as all times. (Replaces immediately when used.). Empties incontinent pad barrel and linen containers every shiftand replaces plastic bags in containers. Reports at end of shift to oncoming CNA’s. Understands and agree to follow policy of no transfers untilCertified. Agrees to obtain assistance from a CNA assigned toassist the Care assistant with tasks they are not comfortable ortrained to perform and with ALL transfers.

Job Requirements:

Must be 18 years of age or older and have a high school diplomaor equivalent. Must be a graduate of a NA1 training program. Must have passedthe program and must pass state test within 4 months fromcompletion. Must be willing to care for geriatric patients and have agenuine concern for their welfare. Must be willing to receive, understand, and followorders. Must be willing to work and cooperate well with other nursingstaff, other departments, and families and visitors. Must be kind and respectful to patients, staff, andvisitors. Must be able to use equipment without jeopardizing the safetyof self, others, or equipment and use supplies safely andeconomically. Wears approved colored dress uniform, or approved colored topand natural colored hose, white nonporous (leather) shoes andnametag. Demonstrates neat appearance and good personalhygiene. Must read, know, and follow personnel, department, and Facilitypolicies and procedures. Must attend in service training and other staff meetings asrequired.
