$17.00 – $25.50 DepartmentDetails Support multiple locations within 150 miles of your permanentaddress Mileage reimbursement Additional $5 per hour float premium Additional $1.25 per hour weekend shift differential Additional $1.25 per hour evening/night shift differential;6:00pm – 6:00am Required to work at least 30 hours per month Required to work at least one weekend rotation per month 3-month previous experience in Long Term Care required, 6months preferred No contract required Job Summary The Nursing Assistant (NA)serves as caregiver to the resident during the scheduled workperiod in long term care. Provides resident-centered nursing careand daily living assistance to assigned resident under thesupervision of a registered nurse (RN). Knowledge of and deliversage-appropriate care related to the physical and psychologicalneeds of the resident as per care plan. Considered a member of thenursing team and is expected to know, and will be held accountablefor, following infection prevention and control policies andPersonal Protective Equipment use. Provides assistance with basichealth care needs including daily living activities that mayinclude, but are not limited to, bathing, toileting, grooming,dressing/undressing, obtaining and recording vital signs, andproviding psychosocial support and other personal care to assignedresident. Assists the resident in transferring, repositioning, andwalking using correct and appropriate transfer techniques andequipment and also provides range of motion and passive exercises.

Completes tasks related to food preparation and dining, whilecreating a positive dining experience for the resident. Assistsresidents with meals and snacks, provides water and supplements asappropriate. The NA aids providers and nursing staff members withprocedures, if needed. Documents resident interactions as neededand the outcomes related to resident care, behavior, activity, anddietary. Communicates resident’s changing condition and carerelated concerns/responses to the charge nurse. Collectsnon-invasive body fluid specimens and gathers vital signs. Provideshousekeeping and laundry services to assist residents with keepingliving areas clean and operational. Provides assistance anddocumentation with wellness and recreational programs andencourages residents to participate. May participate indepartmental/organization committees and task forces as needed.

Qualifications High school graduate or general equivalency diploma(GED) preferred, but not required. Minimum age of 16. Priorclinical or nursing assistant experience in long-term carepreferred. Required Certified Nursing Assistant (CNA) certificationwith the State Board of Nursing, or state certified CNA within fourmonths of date into the position by being enrolled in a trainingprogram or in the process of obtaining certification throughEndorsement. CNA training and certification required and providedby facility.

Colorado:

Depending on facility, Qualified MedicationAdministration Person (QMAP) certification with the ColoradoDepartment of Public Health & Environment required. Satisfactorilycompletes yearly department specific competency testing andmaintains a record of continuing education credits for each year. Aminimum of 12 hours continuing education are required annually.

Maintains all department specific required certifications. Whenapplicable, for applicable states, certified and registered withthe Unlicensed Assistive Person (UAP) Registry. Dependent onfacility and required responsibilities, this role may classify as aSanford Category II Driver requiring a motor vehicle report andproof of valid driver’s license. Also must be medically qualifiedto perform the essential driving functions of this position as perour Sanford Driving Policy per the leaders request.
