Independent Living Coordinator 3.5 3.5 out of 5 stars 1726 Kingsley Avenue, Orange Park, FL 32073 Job Summary We are seeking a dedicated and organized Independent Living (IL) Program Coordinator to join our dynamic team. This role is responsible for overseeing the delivery of IL services, ensuring program compliance, and serving as a liaison between case management, community partners, and educational institutions. The IL Program Coordinator will play a critical role in preparing young people for independence by coordinating services, training opportunities, and housing resources. R esponsibilities Identify all youth and young adults eligible for IL services and ensure compliance with all program requirements. Serve as the IL consultant and support resource for case management staff, community providers and agencies involved in IL services. Review IL documentation for accuracy, completeness, and timely submission. Obtain annual credit checks for all foster youth aged 14 and older. Facilitate and lead group life skills trainings. Create and maintain young adult case files and provider entries in the Florida Safe Families Network (FSFN) system. Communicate with school personnel regarding tuition waivers, academic concerns, and school-related supports. Monitor eligibility for Extended Foster Care (EFC) and Postsecondary Education Services and Supports (PESS); track annual redeterminations Serve as the agency liaison with the Agency for Persons with Disabilities (APD) and ensure timely submission of waiver and legal guardianship applications. Attend statewide IL-related conference calls, trainings, and events. Perform other duties as assigned. Q ualifications Bachelor’s degree in Human Services or a related field required; Master’s degree preferred. Minimum of two years of experience working with at-risk children and families. Strong interpersonal, communication, presentation, and organizational skills. Proficient in basic computer skills and data entry. Child Protection Certification or certification eligibility required. Must successfully complete Level 2 background screening. Valid Florida driver’s license, clear driving record, and proof of insurance required.
