Position Purpose:

Develops, assesses, andfacilitates complex care management activities for primarily mentaland behavioral health needs members to provide high quality,cost-effective healthcare outcomes including personalized careplans and education for members and their families related tomental health and substance use disorder.

• Evaluates the needs ofthe member via phone or in-home visits related to the resourcesavailable, and recommends and/or facilitates the care plan/serviceplan for the best outcome, which may include behavioral health andsocial determinant needs
• May perform telephonic, digital, homeand/or other site visits outreach to assess member needs andcollaborate with resources
• Develops ongoing care plans formembers with high level acuity and works to identify providers,specialists, and community resources needed for care includingmental health and substance use disorders
• Coordinates asappropriate between the member and/or family/caregivers, communityresources, and the care provider team to ensure identified servicesare accessible to members
• Monitors care plans/service plansand/or member status and outcomes for changes in treatment sideeffects, complications and clinical symptoms and providesrecommendations to care plan/service plan based on identifiedmember needs
• Facilitates care coordination and collaborates withappropriate providers or specialists to ensure member has timelyaccess to needed care or services
• Collects, documents, andmaintains member information and care management activities toensure compliance with current state, federal, and third-partypayer regulators
• Provides education to members and their familieson procedures, healthcare provider instructions, treatment options,referrals, and healthcare benefits, which may include behavioralhealth and social determinant needs
• Provides feedback toleadership on opportunities to improve and enhance care and qualitydelivery for members in a cost-effective manner
• Performs otherduties as assigned.
• Complies with all policies and standards.

Education/Experience:

Requires a Master’s degree in BehavioralHealth or Social Work or a Degree from an Accredited School ofNursing and 2

• 4 years of related experience.

License/Certification:

Licensed Master’s Behavioral HealthProfessional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based onstate contract requirements with BH experience required ForSunshine Health (FL)

Only:

Employees supporting Florida’sChildren’s Medical Services must Hold a Master’s degree in socialwork or related field and have one year of related professionalpediatric care experience. May require up to 80% local travelrequired

Pay Range:

$55,100.00

• $99,000.
