Dependency Case Manager Pre-Independent Living 4.1 4.1 out of 5 stars Sebastian, FL JOB St. Johns County Board of County Commissioners is seeking a Pre-Independent Living Coordinator for our well-established Community Based Care Program, the Family Integrity Program, which is a division of the St. Johns County Health and Human Services Department whose headquarters are located in beautiful St. Augustine, Florida. The St. Johns County Family Integrity Program’s mission is to work in partnership with the State of Florida and the local community to develop, implement and manage a highly effective, strength-based community-based system of care for abused and neglected children and their families. The Family Integrity Program provides Dependency Case Management, Family Preservation, Outreach, Foster Care Licensing, Adoption and Independent Living Services for youth aging out of foster care. Employees work in a team environment across the various programs with a consistent focus on child safety, child well-being and permanency.
EXAMPLE OF DUTIES
The ideal Pre-Independent Living Coordinator candidate must possess St. Johns County’s core values of Dedication, Openness, Adaptability, Visionary, Fiscal Responsibility, Integrity, and Humility and must work well in a collaborative team environment. This position is responsible for performing case management duties in support of assigned clients and programs. Additionally, this position is responsible for: Support, train and assist caregivers with providing skills youth need to successfully transition to adulthood. Assess youth’s progress with life skills and incorporate outcomes into ongoing case management planning.
Complete monthly informal needs assessment for youth 13 years of age and older.
Encourage and support youth aged 16 and older to participate in an Independent Living Needs Assessment.
Assist youth aged 16 and older with the development of an Independent Living Skills Plan.
Identify resources to address areas needing improvement and develop a plan to increase the youth’s skills. Complete referrals pertaining to life skill development, as needed. Provide life skills directly to youth, when needed. Document the assessment and progression of each youth’s life skills and the Florida Safe Families Network (FSFN). Assist in determining appropriate case plan tasks and reporting updates on the progress of these tasks to the primary case manager so it can be included in each Judicial Review. Develop and maintain Transition Plans for youth to assist them with determining short-term and long-term goals. Maintain and prepare complete and accurate case records and reports. Participate in supervisory review of cases with ILCS at least quarterly. Collaborate with each child’s system of care team to develop and recommend appropriate plans of service. . Assist clients by coordinating or providing transportation to needed services.
Conduct home, school and work visits, as needed.
Network with local organizations to develop partnering agencies willing to hire and/or mentor teenagers. Work closely with other departments to ensure caregivers of teenagers understand their roles and responsibilitiesProvide support to the Independent Living Coordinator, as needed Identify and screen youth for interest in participation in a Youth Council. Maintain all required documentation in FSFN.Maintain travel logs and attend training as needed.
Perform other duties as assigned.
