We are seeking an Assistant Director of Wellness to join our team. The ideal candidate will work alongside the Director of Nursing with a common goal of providing exceptional care to our residents. This position will spend some of their time on the floor and some of their time with leadership duties. LPN and Assisted Living experience is required. Responsibilities Provide coverage on nursing cart. Develops and maintains a working knowledge of all policies and procedures pertaining to the position, the department, and the personnel. Responsible for the outcome of inspections as it pertains to wellness department. Develops and implements active wellness program for residents, working with Life Enrichment Director. Communicates with the medical community as necessary on resident’s behalf. Accepts physician’s orders, transcribes and follows through to ensure orders are carried out. Reviews physician’s orders and medication administration forms to ensure they are correct and accurate monthly. Maintains resident files in an orderly manner and documents all pertinent information regarding residents. Orders supplies and maintains expenses within budget limits. Prepares residents charges and reports to Director for approval and billing. Attends or completes a minimum of 12 hours of annual training on topics relevant to the community’s resident population. Provides on call coverage in the absence of the Wellness Director. Performs other duties as assigned.
Residents:
Assesses resident within 90 days prior to move-in and annually throughout residency or as the resident’s condition changes. Develops the initial Individual Service Plan (ISP) to meet the needs of the resident within 72 hours of move-in and the comprehensive ISP within 30 days of move-in and annually or as the resident’s condition changes. Oversees resident care provided by the wellness department. Provides hands-on care as needed. Investigates all resident and family complaints or concerns regarding the wellness department and reports findings to the Executive Director. Acts to ensure problems are resolved. Maintains contact with residents when they are in the hospital or nursing home on a temporary basis and keeps abreast of their condition through the community’s medical staff. Notifies Executive Director of these absences. Works with Life Enrichment Director to plan and implement special activities for assisted living residents and residents with special needs, ensures follow through by wellness team.
Staff:
Create wellness staffing schedule. Interview, hire, train, and evaluate all wellness team members. Manages performance related issues and follow up. Manages timesheets for wellness team, ensuring there are no missing clock ins/outs, pto requests, etc. Conducts monthly in-service training for wellness team. Conducts related in-service training for all team members on topics such as proper liŌing procedures, training for bloodborne pathogens, etc. May supervise and/or conduct other training sessions for team members as needed. Provides first aid to injured team members and maintains OSHA log.
Requirements:
Must have current State of Florida Licensed Practical Nurse license. Must complete the continued competency requirements for renewal of an active Licensed Practical Nurse license according to the State of Florida and maintain original documentation of completion. Must be able to stand and walk for extended periods of time. Must be able to exert up to 75 lbs. of full force occasionally and exert up to 50 lbs. of force frequently to lift, carry, push, pull or otherwise move objects including the human body. Must be available for on-call, nights, holidays, weekends, and emergencies. Must have two years of nursing experience, preferably in long term care. Must have comprehensive knowledge of nursing principles and practices. Must have the ability to apply principles of management and human relations. Must be neat and professional in dress during work hours. Must be CPR and First Aid Certified. Must have basic computer skills and be able to learn and operate required programs. Must have strong customer service skills and be able to teach those to wellness team.
