The Wellness Director will ensure the provision of high-quality resident care, promote health and wellness, conduct regular visits with each resident, assess their needs, and maintain open communication with residents, their representatives, and the care team. The Wellness Director plays a crucial role as part of the community’s management team. This role reports to the Executive Director.
Qualifications:
Current Georgia Licensed Practical Nurse (LPN) or Registered Nurse (RN) license. Must maintain CPR certification. Minimum of one year of experience working with residents in a long-term care setting. Ability to respond to questions and emergencies during off-duty hours as scheduled. Ability to build relationships and interact effectively with older adults. Desire to work with older adults. Supervisory experience in healthcare preferred. Must be 21 years of age and pass a satisfactory criminal history check. Must undergo a physical exam by a licensed physician and pass a negative drug screen. Must be able to react in emergency situations.
Primary Responsibilities:
Conduct regular health and wellness checks with each resident and address any health-related questions or concerns. Perform semi-annual assessments for assisted living residents and quarterly assessments for memory care residents. Review resident information and assess their condition prior to move-in, during changes in their condition, and as required by Prema policy and procedures and state regulations. Review medical reports of new residents to ensure the facility can meet their individual needs and fulfill regulatory requirements. Develop and maintain personalized Negotiated Service Plans for each resident. Ensure resident information is properly entered into the Emergency Call System and provide necessary records for resident emergency transport as per Prema policies and procedures. Oversee medication schedules and order medications for residents receiving medication assistance services, following state, federal, and Prema guidelines. Maintain accurate documentation of physician’s orders, medication assistance, injuries, incidents, and changes in resident status or care levels. Act as a first responder in emergency situations when onsite. Maintain ongoing communication with residents, their families, staff, and medical providers regarding their medical needs. Coordinate additional health services as necessary. Foster positive relationships with Home Health Agencies, Hospice Agencies, Vendors, Physicians, and other Resident Services Providers. Assist in coordinating residents’ dietary needs with the Food Service Director. Be on call to address medical questions and resident emergencies as per schedule. Mentor and train resident care employees in accordance with Prema policies and procedures, in-service training programs, and state and federal regulations to ensure regulatory compliance. Supervise caregiver staff to ensure high-quality care that aligns with the Prema philosophy of Person-Centered Lifestyle and resident rights, policies, and state regulations. Ensure compliance with infection control guidelines. Maintain residents’ wellness files, ongoing assessments, and oversee care staff documentation.
