JOB SUMMARY
The role of the Care Coordinator (CC) is to provide hands-on care and support to elderly, independent living residents while promoting independence. The CC will provide services to residents that are offered and advertised within the IL marketing materials. This is included, but not limited to, providing meal service, conducting meal deliveries, in-room or off-site (ALF) laundry, showers, medication support, pet assistance, mail collection/delivery or other as-needed ADL support. Due to the personal nature of the job, the CC should have good people skills, a compassionate spirit, and an enjoyment in helping others.
JOB DUTIES
Welcome guest and show property to prospective residents Provide quality care and assistance to residents in accordance with the resident’s preferences and company policies. Assist residents in meeting their physical, emotional, and spiritual needs. In the absence of the housekeeper, conduct housekeeping as scheduled or needed. Each resident can elect housekeeping services either weekly, bi-weekly, or once per month. Participate in daily housekeeping of the community center and business offices. Maintain the housekeeping cart supplies/equipment and notify the property manager of ordering needs. Assist residents who have enrolled in optional/add-on services such as bathing, medication delivery, laundry, changing bedding, mail delivery, pet or plant care or other individualized services. Check on residents with defined/daily safety checks and document activity. Encourage residents to attend meals and developing, planning , implenting activities in the community center, escort residents to and from the dining room via walking or golf cart if needed, assist with serving meals, assist residents with meals in ways that meet their individual needs and preferences while promoting independence, observe and report changes in the residents’ appetite and physical abilities to eat and drink. Assist with cleaning the dining room after the meal service as assigned. Encourage resident participation in programs, socialization, and activities by accompanying them and providing encouragement and positive reinforcement. Lead or support activities hosted in the community center and/or support others leading activity events. Communicate maintenance needs to the property manager and maintenance director and follow up to ensure completed/resolved timely. Walk property to ensure safety and cleanliness of the community exterior spaces, report concerns to appropriate parties such as Property Manager, Maintenance, etc. Establish and maintain good relationships with residents and their families. Practice proper body mechanics when lifting and follow safety guidelines according to residence policies and procedures. Respond to resident emergencies following proper policy and procedure. Protect and maintain resident rights and privacy while encouraging and supporting resident independence. Treat residents, vendors, families, and co-workers with a professional, courteous, and helpful attitude. Spend time with and get to personally know each resident to foster relationships with them and their family members. Adheres to The Landing Senior Living Employee Handbook, the Employee Code of Conduct & all policy and procedures set forth by the company. Perform other projects or duties requested by management.
KNOWLEDGE, SKILLS, ABILITIES & JOB REQUIREMENTS
Weekend rotation and holiday participation required. After-hours, emergency phone participation/rotation schedule with other managers required. Housekeeping or Current Certified Nursing Assistant (CNA) Certification preferred but not required. Experience providing care to the elderly. Ability to relate and work well with various individuals/personalities. Positive attitude and energy to coach and encourage resident participation. Ability to use good judgement and make decisions in the best interest of resident safety and well-being of the resident and the company. Ability to multitask and properly document all aspects of resident care delivery. Physical abilities demanded by the nursing profession to include aiding with patient transfers, administering treatments or medications, lifting/moving medical supplies, walking, standing, sitting for long periods of time and some car travel between locations. CPR/First Aid Certification. Attend all required staff training, in-service, orientation & meetings. Friendly, caring disposition and desire to work worth with older adults. Valid driver’s license and reliable transportation. TB Skin Test, Covid Screen, criminal background check and drug screen required. The Landing Senior Living is an equal opportunity employer.
