Eliot is seeking motivated Bilingual (Spanish) Care Managers to join our Community Behavioral Health Center (CBHC) which serves as a single entry point for mental health and substance use services for youth, families and adults.
The ideal candidate will provide in-home support and services to clients, primarily focusing care coordination and ensuring that treatment goals are culturally competent, realistic, attainable and promote recovery.
Responsibilities:
Provide client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement
Deliver interventions in the office, virtually, in the community, and at clients’ residences based on client preferences, needs, and clinical direction
Enroll eligible individuals in care management programs as needed. Assist the team and clients in ongoing assessment of symptoms and responses to interventions
Contribute to the development of treatment plans as part of the client’s treatment team
Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery
Ensure each individual is actively involved in their treatment planning process, and their preferences are incorporated
Coordinate and ensure timely face-to-face contact with enrollees in the community following inpatient discharge.
Qualifications:
Bachelor’s degree in Psychology, Social Work or related field
3 year experience working with adults with mental illness and/or substance abuse
Fluent in Spanish and English
Valid Massachusetts Driver’s License
Schedule:
Monday-Friday, 9am-5pm with remote/hybrid flexibility(3 days on-site, 2 days remote)
Annual Salary
$53,000
- $53,000 USD
