Telehope Care Manager – St. Louis City Provident Inc / Mary Ryder Home – 3.5 St. Louis, MO Job Details Full-time $19.23 – $21.63 an hour 1 day ago Qualifications Crisis intervention HIPAA Mid-level Microsoft Office Bachelor’s degree Counseling Computer skills Social Work 1 year Full Job Description Essential Duties & Responsibilities Accountabilities Provides telephonic support and case management to older adult client’s ages 55+ Timely service delivery once referrals are received, including timely communication with referring party and contacting referred clients Achieve target number of client interactions and services per month Timely completion of documentation in Provident’s EHR Coordinating care and referrals with partner agencies Provide presentations and present at community outreach events to raise awareness of program and increase referrals. Routine attendance and participation in supervision with Associate Director Awareness of community resources appropriate for the population being served. Coordination with Provident’s other programs to link clients with more intensive interventions, such as psychiatric services, counseling, and 24/7 telephonic crisis interventions Participate in ongoing training for supporting older adults Completing regular outcome measures with clients to evaluate progress in the program Attend audits or meetings with community partners to address progress and barriers to the program. Work Conditions and Environment Work schedule is a 40-hour work week, with typical hours being between 8:00 am and 6:00 pm. Some evenings may be required to accommodate community meetings or client phone calls. Reliable transportation to travel to other Provident sites and community-based sites, when needed. Must have stamina to provide live, in-person presentations in small to large group settings for 30+ minutes This position is occasionally out in the community to meet with other groups of individuals. Must be able to navigate community-based settings independently. Must have a reliable high-speed internet connection and reliable phone service to work remotely. Must be able to lift up to 25 lbs. Have the ability to sit for long periods of time.
Job Requirements and Qualifications Minimum Education:
Bachelor’s degree in social work, counseling, psychology, nursing or related helping field.
Minimum Experience:
1 year of experience in the helping field
Licenses/Certifications:
Complete Applied Suicide Intervention Skills Training (ASIST) within first 90 days of employment. (Training provided on-site at Provident) Complete at least 14 hours in training on working with older adult populations within the first 6 months of employment. Other Skills Required Ability to work with a variety of clients who are 55+ and have varying mental/behavioral health concerns Ability to effectively communicate information and ideas, both orally and in writing, with both clients, colleagues, grantors, funders, auditors, and other collaborative agencies. Knowledge of a variety of coping and coaching skills to help clients reduce anxiety and depressive symptoms. Knowledge of crisis intervention, suicide risk screening and assessment, and safety planning techniques. Knowledge of community support services and resources. Knowledge of HIPAA and HITECH rules. Strong skills using and navigating computers and software programs. Ability to navigate computer, phone, electronic health record (EHR), Microsoft Office Suite, and other information technology systems independently. Skilled in preparing and maintaining client documentation in an accurate and timely manner, in compliance with established audit and agency standards. Participate as a team member, collaborating with and providing excellent customer service to clients, funding sources, peers, other Provident employees, and other internal and external customers. Serve as a representative of Provident, displaying courtesy, tact, consideration and discretion in all interactions with Provident team members, clients, grantors, funders, auditors, and members of the general public. Ability to foster a positive and inclusive work culture with peers and inclusive treatment environment for clients. Ability to maintain healthy client boundaries The Telehope Care Manager will be responsible for follow up care for older adults residing in St. Louis City who have been identified as being at risk of depression, anxiety, and other mental health issues. This role will provide telephonic screening and routine follow-up support calls to build coping skills, problem solve, providing resources to the older adult and their caregiver, and encourage linkages with their community and other natural supports. Each older adult client will be provided support for up to 6 months or more by this position. This position will collect outcome measures to track each client’s progress in the program. They will also be responsible for timely completion of documentation to track each interaction with clients in our Electronic Health Record (EHR). This role is instrumental in launching this new program for Provident, which will help older adults in St. Louis City connect with mental health services in a way that is accessible and meaningful for this population.
