Care Manager 3.4 3.4 out of 5 stars Natchez, MS Job Overview The Care Manager will hire and onboard new CG’s, Create and maintain client and caregiver schedules, perform monthly audits, perform quarterly home visits, schedule nurse assessments, check insurance eligibility, problem solve issues with internal departments such as payroll, billing, renewals, and intake. Occasional community outreach such as health fairs and business expos. Light marketing to local clinics and hospitals. CG performance evaluations, assisting clients fill out packets to become eligible for PC services, keeping track of excel word documents and spreadsheets. Responsibilities Conduct comprehensive assessments to identify client needs and develop tailored care plans. Coordinate services across multiple agencies to ensure clients receive appropriate support. Provide guidance and mentorship to clients, empowering them to achieve their goals. Facilitate crisis intervention strategies for clients in need of immediate assistance. Monitor client progress and adjust care plans as necessary to meet evolving needs. Collaborate with healthcare providers, social workers, and community organizations to enhance service delivery. Maintain accurate documentation of client interactions and progress in compliance with regulatory standards. Engage in workforce development initiatives to improve service delivery within the organization. Skills Strong knowledge of Medicare policies and procedures. Experience in child welfare practices and principles. Proficient in mentoring individuals from diverse backgrounds. Background in addiction counseling and behavioral health services. Effective crisis intervention skills with the ability to remain calm under pressure. Supervisory experience, demonstrating leadership capabilities within a team setting. Familiarity with childcare practices and developmental milestones. Knowledge of workforce development strategies aimed at enhancing client employability. Proficient in motivational interviewing techniques to encourage client engagement and self-efficacy. This role is essential for fostering a supportive environment that promotes positive outcomes for clients while ensuring they have access to the necessary resources for their journey toward improved health and well-being.
