EST Position Purpose:
Assists indeveloping, assessing, and coordinating holistic care managementactivities to enable quality, cost-effective healthcare outcomes.
May develop or assist with developing personalized service careplans/service plans for long-term care members and educates membersand their families/caregivers on services and benefits available tomeet member needs. “Evaluates the needs of the member, theresources available, and recommends and/or facilitates the plan forthe best outcome Assists with developing ongoing long-term careplans/service plans and works to identify providers, specialist,and/or community resources needed for long-term care Coordinates asappropriate between the member and/or family/caregivers and thecare provider team to ensure identified services are accessible tomembers Provides resource support to members and theirfamilies/caregivers for various needs (e.g., employment, housing,participant direction, independent living, justice, foster care)based on service assessment and plans Monitors care plans/serviceplans, member status, and outcomes, as appropriate, and providesrecommendations to care plan/service plan based on identifiedmember needs Interacts with long-term care healthcare providers andpartners as appropriate to ensure member needs are met Collects,documents, and maintains long-term care member information and caremanagement activities to ensure compliance with current state,federal, and third-party payer regulators May perform home and/orother site visits to assess members’ needs and collaborate withhealthcare providers and partners Provides and/or facilitateseducation to long-term care members and their families/caregiverson procedures, healthcare provider instructions, service options,referrals, and healthcare benefits Provides feedback to leadershipon opportunities to improve and enhance the quality of care andservice delivery for long-term care members in a cost-effectivemanner Performs other duties as assigned Complies with all policiesand standards. Managing a case load for healthcare members withlong term care needs. Geriatric long term care Member assessmentsand notes. Complete assessments with members, caregivers, orproviders to obtain information regarding client status, supportsystem, and need for services for care plan development. Monitordelivery of services and follow-up with members, caregivers, orprovider s through in person visits and telephonic contactAuthorize and coordinate referral for services. Ensure providerservices are delivered without gaps and identify functionaldeficiencies in plans of care. Assist in coordinating thedevelopment of informal or voluntary services to integrate into themember care plan Collaborate with discharge planners, physicians,and other parties to ensure appropriate discharge plan, care plan,and coordination of acute care and long-term care services! Assistmember with filing and resolving complaints and appeals.
