Tailored Care Manager for our TCM Program.

Qualifications & Experience:

A minimum of a bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN); and Two years of experience working directly with individuals with behavioral health conditions. Preferred experience includes working with adults and children with MH and/or SUD needs Primary Responsibilities- The primary responsibilities of the care managers are to provide whole person integrated care management services to members who are enrolled in the Tailored Plans. Provision of Care Management Services- The following activities will be completed by the Care Managers via in person and telephonic support of their clients: Care Management Comprehensive Assessment Care Plans Development of the Care Team Care Coordination 24/7 On Call Crisis Support (on call coverage is rotational)
