Description At the beginning of every shift, walk the entire unit to ensure that there are no problems, incidents nor issues that need review or immediate attention, including resident care and/or behavioral concerns.
Advocate for and drive engaging and holistic parallel programming within the neighborhood at all appropriate times to connect with residents through mind, body, and spiritual channels, as each pertain to individual residents.
In conjunction with the Service Plan Team, create a person-centered service plan on move-in, at 30 days, for changing care needs and at least every 6 months thereafter.
Participate in weekly care plan meeting and help drive focus on person-centered care.
Meet regularly with the Director of Wellness, Director of Case Manager/and or Assistant Director of Wellness to exchange information about the Residents and personal care services.
Ensure the delivery of optimal quality care to the Residents following Company Policies.
Evaluate care being given to the Residents. Coordinate care with the Executive Director/General Manager, Director of Wellness, Director of Case Management and/or Assistant Director of Wellness.
Assess Residents regularly to establish their needs and to establish a highly personalized care plan.
Manage and ensure the care team’s use of the behavior tracking log.
Partner with Lifestyle Director to ensure a variety of appropriate group programming, individualized single or small group centered programming, and/or life skill-based programming is available to residents via both the Programming and Reflection care teams (RSAs); ensure care team understands how to employ these programming methodologies and ensure RSAs are engaging residents programmatically, through assignments if necessary. Directly support Reflections programming by leading occasional resident programs throughout the week.
Provide input to the Lifestyles Director regarding the monthly calendar of activities and review calendar to ensure it meets the needs of the residents.
Regularly be present during resident meal service (when in the community), partner with food and dining teams, and meet routinely with Director of Food Services to advocate for the best dining experience possible for Reflections’ residents.
Assist in assessing prospective Residents for Reflections Program.
Provide input regarding admission, retention, and discharge of Residents
Communicate with healthcare providers regarding residents’ needs/concerns.
Interview, hire, train, mentor, evaluate, promote, schedule, and terminate employment of Reflections’ personnel in conjunction with Director of Wellness.
Assist the Director of Wellness and/or designee with scheduling Reflections’ staff, assuring that one RSA on each shift has basic First Aid Certification.
Have a complete understanding of the medication system adhering to federal, state, local Department of Health laws and regulations and Company policies.
Assist the Director of Wellness with monitoring the medication assistance provided by trained/certified medication aides including medication supervision, medication records, medication re-ordering and medication accountability.
Direct Resident Service Aides in all aspects of care, including behavioral expression support and intervention, approaches, and strategies.
Schedule and conduct regular in-services to improve the quality of care by Reflections’ personnel.
Serve as community’s subject matter expert (SME) for all topics related to supporting residents living with memory loss or other forms of cognitive impairment via a personalized, empathetic approach and plan of care.
Serve as SME for community dementia-focused trainings, including but not limited to leading memory care sections of New Hire Orientation training and maintaining monthly and annual in-services schedules to meet Department of Health requirements.
Keep abreast of professional developments in the Alzheimer’s and other dementias field, including maintaining current Alzheimer’s Association essentiALZ certification.
Relate to and give support to family members on an ongoing basis.
Conduct monthly support group for Residents’ family members.
Maintain open communication with responsible parties and families regarding needs and status of Residents.
Maintain on-going infection control surveillance, OSHA standards, HIPAA regulations and basic sanitation and safety.
Maintain active community and professional relationships to support positive promotion of company.
Fully understand and be able to implement emergency procedures as outlined in Company Emergency and Disaster Plan.
Complete scheduled checks of Residents’ rooms.
Assist the Director of Wellness in ensuring that monthly weights are taken and recorded.
Implement and supervise the systems for maintaining accurate records and documentation that meets all requirements of the facility and DOH regulations.
Maintain regular case notes on Reflections’ Residents and document significant events/incidents in compliance with company policy.
Help maintain personnel and resident files.
Performs other duties as assigned by the Executive Director/General Manager. Qualifications
