If so, we want YOU to join our team! We’re currently seeking a Part-Time Service Coordinator to support participants in the Nursing Home Transition and Diversion (NHTD) Waiver Program . This vital program helps individuals who qualify for nursing home-level care remain in their own homes and communities, with the support they need to thrive.

What You’ll Do:

As a Service Coordinator, you’ll work directly with older adults and individuals with disabilities to help them: Live independently in the community Access essential services and supports Navigate Medicaid and healthcare systems Maintain dignity, autonomy, and safety

Key Responsibilities:

Coordinate Services:

Develop and implement Person-Centered Service Plans that promote health, independence, and quality of life.

Conduct Assessments:

Identify participant needs through comprehensive evaluations to determine appropriate supports and services. Advocate for

Participants:

Help individuals navigate Medicaid, community resources, and complex healthcare systems.

Monitor Progress:

Regularly check in with participants and adjust service plans as needs change.

Foster Relationships:

Build collaborative partnerships with participants, families, caregivers, and service providers.

Ensure Compliance:

Maintain accurate, timely documentation that aligns with state and federal program regulations. Minimum Qualifications

• NYS Licensed Licensed Clinical Social Worker (LCSW)
• NYS Licensed Master’s or Doctoral Degree in Social Work or Psychology Licensed Physical Therapist
• NYS Licensed Registered Professional Nurse (RN)
• NYS Licensed Licensed Occupational Therapist
• NYS Licensed Licensed Speech Pathologist
• NYS Licensed Certified Rehabilitation Counselor (CRC) Certified Teacher of Students with Disabilities
• NYS Certified AND At least 1 year of experience providing case management, service coordination, or similar services to individuals with disabilities and/or Traumatic Brain Injury (TBI)✅ Path 2: Degree + Experience-Based If you don’t hold one of the above licenses or graduate-level degrees, you can still qualify by meeting both of the following: A Bachelor’s Degree in any field + 3 years of relevant experience OR An Associate’s Degree in any field + 5 years of relevant experience Relevant experience includes work in: Case management Service coordination Information and referral services Service brokering for individuals with disabilities and/or TBI Why Join Us?
