We believe in compassionate, person-centered care—and we’re looking for a driven Care Manager Supervisor to lead a dynamic team focused on transforming lives. The Opportunity As the Care Manager Supervisor, you’ll play a critical leadership role in supervising care coordination staff who serve individuals with chronic health conditions, behavioral health needs, and social challenges. You’ll ensure quality care delivery, regulatory compliance, and staff development—all while driving impactful outcomes for our Health Home members. What You’ll Do Supervise a team of care managers, providing coaching, oversight, and performance management. Ensure documentation, care planning, and service delivery meet high standards. Lead team huddles, case conferences, and individual supervisions. Monitor caseloads, performance metrics, and follow-ups after critical events (hospitalizations, incarcerations). Collaborate with internal/external providers to ensure seamless care coordination. Use EHR and data dashboards to guide decision-making and continuous improvement. Support onboarding, training, and staff engagement initiatives. Serve as an operational leader and act on behalf of the Clinical Director when needed. What You BringEducation Bachelor’s degree with 3+ years of experience, OR Master’s degree (MSW, MHC, MPA, etc.) with 1+ year of relevant experience Experience 1+ year of supervisory experience in care coordination or behavioral health Experience with high-risk or complex populations strongly preferred Skills Motivational leadership style Excellent documentation, time management, and multitasking abilities Proficient in Electronic Health Records and Microsoft Office Strong interpersonal and problem-solving skills Why Join Care Collab?
