DESCRIPTION OF WORK
In this Aging Care Manager 3 position, your primary responsibility is the assessment of risk and the development of care plans to eliminate or reduce the risk for persons reported in need of protective services. Work begins with obtaining information from consumers, caregivers, family members, neighbors and others in order to complete an accurate assessment. Using all available information, you will make a determination if need is substantiated or unsubstantiated. Communication is key as you develop a care plan with the consumer to solve both presenting and underlying problems and arrange for necessary services. Frequent home visits and telephone contacts will allow you to monitor consumers’ behavior and case plan effectiveness. Included in your duties will be investigating sites for clients at risk and initiating procedures for court related intervention and guardianship cases. You will be expected to maintain and complete detailed case information in a timely manner. You will have the opportunity to participate in supervision, scheduled meetings, annual enrichment and other training as required.
Work Schedule and Additional Information:
Full-time employment
Work hours are 8:30 AM to 4:30 PM, Monday•Friday, with 30-minute lunch.

You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
One year as an Aging Care Manager 2 or Aging Program Assessor; or
One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

Legal Requirements:
You must pass a background investigation.
This position falls under the provisions of the Child Protective Services Law.
Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
