THE POSITION

The Luzerne County Area Agency on Aging is seeking dedicated, patient, and professional individuals to join our amazing team and make a difference in the lives of our senior citizens— today!
OF WORK
As an Aging Care Manager 2, you will be responsible for coordinating and overseeing the various aspects of care for your clients. You will conduct field visits to respond to emergency situations, monitor services as specified in the care plan, and provide emotional support to clients while resolving any issues that may arise. ly, you will oversee the financial management of clients who are unable to do so on their own. This may involve setting up budgets with client participation or arranging for an agency to manage their public benefits. You will also be expected to assist clients in navigating the aging service system and help them with pre-placement and placement activities ifmunity services are no longer sufficient to maintain them in their homes. These tasks are crucial in ensuring the safety and well-being of your clients and require a strong sense of responsibility andpassion.

Work Schedule and Information:

Full-time , 37.5 hours per week
Work hours are 8:00 AM to 4:00 PM, Monday – Friday, with a 30-minute lunch.

You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successfulpletion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalentbination of experience and training.
