The Care Manager will play a vital role in coordinating patient care, ensuring that individuals receive comprehensive support throughout their treatment journey. This position is ideal for someone with experience in case management and a strong understanding of medical terminology, as well as familiarity with behavioural health. The Care Manager will work closely with patients, families, and healthcare providers to facilitate effective communication and care planning. Responsibilities Conduct thorough patient assessments to determine individual needs and develop personalized care plans. Coordinate care across multiple disciplines, ensuring that all aspects of the patient’s treatment are addressed. Collaborate with healthcare professionals, including social workers and emergency medicine specialists, to provide holistic support. Maintain compliance with HIPAA regulations while managing sensitive patient information. Provide education and resources to patients and families regarding treatment options and available services. Monitor patient progress and adjust care plans as necessary to meet evolving needs. Offer support in addiction counseling as needed, guiding patients through recovery processes. Document all interactions and updates in accordance with organizational policies. Requirements Bachelor’s degree in social work, nursing, or a related field; relevant certifications preferred. Experience working within a behavioural health is highly desirable. Strong knowledge of medical terminology and case management principles. Familiarity with HIPAA regulations and commitment to maintaining patient confidentiality. Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations. Proficiency in patient assessment techniques and the ability to develop actionable care plans. Ability to work collaboratively within a multidisciplinary team environment. Compassionate approach towards patient care with strong problem-solving skills.
