Care Manager 3.4 3.4 out of 5 stars 44 South Main Street, Randolph, VT 05060 Gifford Health Care is seeking a Nurse Care Manager to join our Community Health Team. The Nurse Care Manager is a grant-funded position within Gifford’s Community Health Team, focused on delivering care management for patients identified as high-risk or high-complexity. This role works closely with patients across the care continuum, using motivational interviewing and behavior change techniques to help individuals set meaningful goals and create action plans that support wellness and encourage personal accountability in maintaining health. The Nurse Care Manager often takes the lead in coordinating a patient’s care, collaborating with providers, clinic staff, and extended Community Health Team resources to support individuals with or at risk for chronic health conditions. Working in partnership with both patients and providers, this position helps build individualized care plans that support physical and mental health, enhance quality of life, and promote self-management. This role is part of the Vermont Blueprint for Health, a statewide initiative aimed at improving population health, integrating care, and managing healthcare costs through prevention, care coordination, and chronic condition management. The Nurse Care Manager is expected to uphold strict confidentiality regarding all patient information and comply with Gifford Health Center’s policies and procedures.
Requirements:
