Job Summary The Care Manager is responsiblefor assisting in the development of discharge planning and carecoordination of post acute services for the hospitalized patient.

Post acute services include DME equipment, home health, hospice,skilled rehab, IV antibiotics, dialysis, nursing home, and homeoxygen. Discharge planning includes daily review of the medicalrecord, communication between nursing, physician, and therapyservices. The case manager must be able to be assertive and selfmotivated as well as have the ability to communicate effectivelywith patients and family members. Must be able to communicateclearly with physicians to ensure appropriate discharge goals aremet. The Care Manager

  • RN is responsible for coordinating andoverseeing discharge planning, transitions of care, and casemanagement activities to ensure optimal patient outcomes. This roleinvolves collaborating with interdisciplinary teams, reviewingmedical records for appropriateness and medical necessity, andmaintaining compliance with federal, state, and accreditationstandards. Essential Functions
  • Conducts daily reviews of medicalrecords to assess the appropriateness of admission, continuedhospital stay, and utilization of diagnostic services.
  • Collaborates with interdisciplinary teams (IDT) to ensure effectivecommunication and coordination of patient care, includingidentifying avoidable days and resolving care transition issues.
  • Develops and implements discharge plans, coordinating post-hospitalplacement and social services to meet patient needs.
  • Refers casesto physicians or managers when patients do not meet establishedcriteria, ensuring timely and appropriate interventions.
  • Servesas a liaison with community agencies, maintaining relationships andfacilitating seamless transitions for discharged patients.
  • Facilitates interdisciplinary meetings to address patient careneeds, resolve challenges, and support collaborative care planning
  • Maintains accurate and timely documentation of case managementactivities, including records of referrals, patient interactions,and compliance with reporting requirements.
  • Identifies andappropriately refers cases to Child/Adult Protective Services,ensuring compliance with legal and ethical standards.
  • Providesprofessional assistance to patients, families, and physiciansregarding discharge planning and post-hospital care options.
  • Performs other duties as assigned.
  • Complies with all policies andstandards. Qualifications
  • Associate Degree in Nursing required
  • Bachelor’s Degree in Nursing preferred
  • 2-4 years of clinicalnursing experience in a hospital, home health, or nursing homesetting required
  • 2-4 years of care management experiencepreferred Knowledge, Skills and Abilities
  • Strong understanding ofcase management principles, discharge planning, and transitions ofcare.
  • Knowledge of federal, state, and Joint Commission standardsrelated to case management.
  • Excellent communication andinterpersonal skills to collaborate effectively with patients,families, and interdisciplinary teams.
  • Ability to assess complexsituations, identify solutions, and implement care plansefficiently.
  • Proficiency in electronic medical records (EMR) anddocumentation systems.
  • Strong organizational and time managementskills to prioritize tasks in a dynamic environment. Licenses andCertifications
  • RN•Registered Nurse•State Licensure and/orCompact State Licensure required•BCLS•Basic Life Supportrequired State Specific Requirements•
Alabama:

Accredited CaseManager (ACM) or Certified Case Manager (CCM) certificationpreferred.
