Columbus, GA This is nota remote or work-from-home position. This position requires you tosit on-site at our Columbus, GA branch location. The RN ClinicalManager coordinates and oversees all direct care patient servicesprovided by clinical personnel. This role is a focused on both homehealth clinical quality assurance and home health clinicaloperations initiatives. Develops, plans, implements, analyzes, andorganizes clinical operations for a specific location managed.
Conducts/delegates the assessment and reassessment of patients,including updating of care plans and interpreting patient needs,while adhering to Company, physician, and/or health facilityprocedures/policies. Manages the assignment of caregivers.
Responsible for and oversees the delivery of care to all patientsserved by the location. Receives case referrals. Reviews availablepatient information related to the case, including disciplinesrequired, to determine home health or hospice needs. Accountable toensure patients meet admission criteria and make the decision toadmit patients to service. Assigns appropriate clinicians to acase, as needed. Instructs and guides clinicians to promote moreeffective performance and delivery of quality home care servicesand is available at all times during operating hours to assistclinicians as appropriate. Assists clinicians in establishingimmediate and long-term therapeutic goals, in setting priorities,and in developing patient Plan of Care (POC). Monitors cases toensure documentation is following compliance with regulatoryagencies and requirements of third-party payers. Ensures finalaudits/billing are completed timely and in compliance with Medicareregulations. Coordinates communication between teammembers/attending physicians/caregivers to ensure theappropriateness of care and outcome planning. Works in conjunctionwith the Branch Director and Company Finance Department toestablish location’s revenue and budget goals. Participates insales and marketing initiatives. Supervises all clinical employeesassigned to a specific location. Responsible for the overalldirection, coordination, and evaluation of the location. Carriesout supervisory responsibilities in accordance with Companypolicies and procedures. Handles necessary employee correctiveaction and discipline issues fairly and objectively, inconsultation with the Human Resources Department and the ExecutiveDirector/Director of Operations. Participates in the interviewing,hiring, training, and development of direct care clinicians.
Evaluates their performance relative to job goals and requirements.
Coaches staff and recommends in-service education programs, whenneeded. Ensures adherence to internal policies and standards.
Assesses staff education needs based on own the review of clinicaldocumentation in addition to feedback and recommendations byUtilization Review staff. Upon completion of the assessment,creates and conducts regular staff education as needed. Analyzessituations, identifies problems, identifies and evaluatesalternative courses of action through the utilization ofPerformance Improvement principles. Responsible for review of theappropriate number of Case Managers and clinical staffdocumentation to include starts-of-care, resumption-of-cares, andre-certifications, for appropriateness of care, delivery, anddocumentation requirements. Responsible for the QA/PI activities.
Works with Utilization Review staff relative to data tracking forperformance review and outcomes of care analysis to determineefficiency, the efficacy of case management system as well as anyother systems and process. Competently performs patient careassignments and staff management activities. Provides directpatient care on an infrequent basis and only in times of emergency.
Acts as Branch Director in his/her absence. Interprets Companystandards and Company policies and procedures to ensure compliancewith external regulatory authorities and ensures that caregiverclinical documentation meets internal standards. Use your skills tomake an impact
Education:
Associate degree in Nursing (ADN)required; Bachelor of Science in Nursing (BSN) strongly preferred.
License/Certifications:
Active and unrestricted Registered Nurse(RN) license or Compact State RN license with authorization topractice in the state of Georgia. Proof of current CPR card and/orBLS certification.
Required Experience/Skills:
Valid driver’slicense, auto insurance and reliable transportation. Two years as aRegistered Nurse with at least one-year of management experience ina home care, hospice or equivalent environment. Home healthexperience is required. Previous experience in home-based carerequired, including home health, hospice, and/or palliative caresettings. Management and people leadership experience is preferred.
OASIS experience is preferred. OASIS certification (HCS-O, COQS,and/or COS-C), preferred. Homecare Homebase (HCHB) experience ispreferred.
CMS PDGM
billing knowledge and/or experience ispreferred.

Annual Bonus Eligible:
Yes,eligible for the annual incentive bonus which has pay-outs bothquarterly and annually. Scheduled Weekly Hours 40 Pay Range Thecompensation range below reflects a good faith estimate of startingbase pay for full time (40 hours per week) employment at the timeof posting. The pay range may be higher or lower based ongeographic location and individual pay will vary based ondemonstrated job related skills, knowledge, experience, education,certifications, etc. $77,200 – $106,200 per year This job iseligible for a bonus incentive plan.
