The Nurse Care Manager in DDHealthcare Home is part of the person-centered integrated care teamand provides individual and population-based health interventionsto persons enrolled in the DD Health Care Home by integrating carefor physical health conditions. This is a Full-time, benefiteligible position working Monday – Friday. This Candidate willcover Cabool or West Plains as well as Howell, Texas andneighboring counties.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

Responsible for the completion of monthly assigned interventions,and all aspects of care according to Department of Mental Healthregulations for the DD Health Home Program for a caseload of up to200 clients Meet with assigned clients to complete health screeninginitially and annually in an office-based or community-basedsetting; identify risk level, care gaps and make clinical carerecommendations Assist in developing a physical health goal for theservice plan; promote healthy lifestyle including healthynutrition, physical activity, and freedom from tobacco use Ensurethat clients are referred to a PCP or specialist, if they do nothave one; provide education to clients/caregivers regarding theimportance of having a PCP Complete metabolic syndrome screeningson assigned clients following the metabolic screening policy andstanding order Perform medication reconciliation for assignedclients who have been discharged from the hospital per agencyguidelines Provide education to clients and caregivers on monthlyreports, and as needed, regarding physical health conditions,treatment, screening, prevention, and self-management Provideconsultation and education for other care team members regardingchronic health conditions Consult with Specialized HealthcareConsultant and Primary Care Physician Consultant as scheduled andas needed Assist in contacting/collaborating with medical providersand other human service agencies regarding significant healthinformation to ensure the provision of holistic services forassigned clients Travel to client’s place of residence as neededMeet service provision expectations per client needs and companyguidelines Will seek culturally competent services and serviceproviders to meet the unique cultural needs of each client Functionas part of a multi-disciplinary team; attend team meetings, andother assigned community meetings Other duties as assigned

WORKINGCONDITIONS & EXPOSURE RISK

Regular exposure to blood and/or bodyfluids Regular exposure to biohazard and/or chemical wastematerials High School/GED required Practical Nursing or Associateof Nursing degree required Bachelor’s degree in nursing preferred

WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

Experience in theIntellectual Developmental Disability (IDD) field preferred A validdriver’s license and agency established minimum automobile coveragerequired

LICENSURE/CERTIFICATION
