The LPN Medication Care Manager isresponsible for providing the highest degree of quality care andservices by administering medication and treatments in a safeorganized manner. Responsibilities include but are not limited toadministration of medications, documentation of medicationadministration, conducting SHUs, and providing resident care whiledemonstrating the Mission for Sunrise Senior Living, “to championthe quality of life for all seniors” in accordance with federal,state/provincial, and local laws, standards and regulations andSunrise Senior Living policies to promote the highest degree ofquality care and services to our residents.
RESPONSIBILITIES &QUALIFICATIONS
Responsibilities:

Medication Administration:
- Provides the highest degreeof quality care and services by administering medication andtreatments in a safe organized manner.
- Reviews, reads, notates,and initializes Daily Log to document and learn about pertinentinformation about residents.
- Receives medication updates fromResident Care Director (RCD) or Wellness Nurse.
- Administers,assists with, and observes medications and treatments for eachresident using the medication administration record and the SixRights of Medication Pass (“Right” resident, medication, dosage,time, route, right to refuse).
- Ensures that medications arepassed according to times.
- Documents and initials as medicationsare given and ensures that appropriate documentation is completedfor refusal or missed doses.
- Maintains confidentiality of allresident information including resident medication among otherresidents.
- Reports all resident concerns made while administeringthe medication to the RCD or Wellness Nurse.
- Restocks medicationcart after all medication passes.
- Assists in checking medicationregardless of packaging system.
- Assesses the residents todetermine need for “as needed medication” and appropriatelydocuments and reports to supervisor.
- Counts all narcotics withanother Medication Care Manager (MCM) or Lead Care Manager (LCM)each shift.
- Maintains and cleans the Medication Room, medicationcarts and treatment carts for neatness, cleanliness, availabilityof medications, and expired medications.
- Follows re-fill processfor medications.
- Helps residents maintain independence andpromotes dignity and physical safety of each resident adhering tothe Sunrise Principles of Service.
- Strives to understand andrespond to each resident with empathy, always remaining mindful ofthe resident’s unique communication patterns, history, and basichuman needs.
- Practices routinely good standard care precautionsof cleanliness, hygiene, and health .
- Audits medication carts.
Resident Care:
- Notifies RCD of any resident and/or familyconcerns through the LEAD process.
- Attends and activelyparticipates in daily Cross Over meetings facilitated by the LCM.
- Conducts Service and Health Updates as directed by RCD.
- Participates in the development of the Individualized Service Plans(ISP)/Individualized Care Plans (ICP) and monthly updates.
- Transcribes orders.
Risk Management and General Safety:
- Partnerswith community team to ensure community is in compliance withOSHA/British Columbia Workers Compensation Act and OccupationalHealth and Safety Regulation requirements, promoting RiskManagement programs and policies, and adhering to safety rules andregulations.
- Practices safety procedures at all times includingPersonal Protective Equipment (PPE), fire extinguishers, SafetyData Sheets (SDS), and Lockout Tagout procedures.
- Reports allaccidents/incidents immediately.
- Reports all unsafe and hazardousconditions/equipment immediately.
- Ensures any cords, carts,equipment, and other hazards are always kept out of the way, notblocking exits and in compliance with fire codes.
- Complies withall infection control techniques, placement of bio-hazardcontainers, removal techniques, procedures, and policies.
- Understands and practices the proper method of attending to anddisposing of, and the possibility of exposure to, blood bornepathogens, bodily fluids, infectious waste, sharp sticks, andhazardous materials.
- Reports occupational exposures to blood,body fluids, infectious materials, sharp sticks and hazardouschemicals immediately.
- Ensures oxygen tanks are stored safely,exchanges guest/resident’s tanks when empty and monitors to makesure liters of oxygen are at prescribed levels.

- Actively participates as a memberof a team and commits to working toward team goals.

- Commitsto serving our residents and guests through our Principles ofService.
- Contributes to the overall engagement programs andprocesses (customer and team member engagement) includingparticipating in the team member engagement survey and engagementimprovement planning workshops.
- Attends regular meetings; TownHall, Department Team, Cross Over, Medication Technician and othersas directed by the Supervisor/Department Coordinator.
- Attendsregular training by RCD and neighborhood coordinators.
- May bedesignated as shift supervisor.
- May supervise other medicationcare managers.
- Maintains compliance in assigned required trainingand all training required by state/province or other regulatingauthorities as applicable to this role to ensure that Sunrisestandards are always met.
Qualifications:
To perform this jobsuccessfully, an individual must be able to perform each essentialduty satisfactorily. The requirements listed throughout this jobdescription are representative of the knowledge, skills, andabilities required.
- LPN, LVN, or state/province specific licensednurse credential.
- In states/provinces where appropriate, mustmaintain certifications.
- Maintain the following certificationsand ongoing training and re-education as required by Sunrise andstate/provincial regulations:
- CPR and First Aid
- Must be atleast 18 years of age.
- Previous experience working with seniorspreferred.
- Desire to serve and care for seniors.
- Ability tomake choices and decisions and act in the resident’s best interest.
- Ability to react and remain calm in difficult situations.
- Ability to handle multiple priorities.
- Possess written and verbalskills for effective communication and level of understanding.
- Demonstrates good judgment, problem solving and decision-makingskills.
- As applicable, all Sunrise team members who drive aSunrise vehicle must sign the Driver job description and understandthe key essential duties for safety and regulatory compliance.
