Work where you matter. A Brief Overview Under the direction offacility leadership this position is responsible for the planningand coordination of direct resident care services. Serves as a coremember of the interdisciplinary client care team. Has the necessaryauthority to assess, plan, evaluate and teach in patient and staffsituations. Carries out administrative and supervisoryresponsibilities as delegated by facility leadership. What you willdo Coordinates admissions, discharges and transfers to and from thefacility. Provides resident care by reviewing physicianprescriptions; communicating the nursing care plan and allpertinent information to other members of the health care team;coordinating with other healthcare providers; providing physical,emotional, psychological, and spiritual support to residents,families, and friends; conferring with physicians. Develops nursingcare plans by obtaining medical histories; examining residents;documenting residents physical condition, psychological status, andinfluencing social factors; conferring with family and friends;developing a nursing diagnosis; establishing nursing careobjectives; defining nursing interventions, schedules, andtimetables. Participates in interdisciplinary team meetings fordevelopment, review and revision of plan of care for resident andfamily. Initiates, participates and coordinates additional meetingsas necessary to address crisis matters and provide support for teammembers. Evaluates resident’s care by adhering to therapeuticstandards; measuring health outcomes against resident carestandards; reviewing resident’s records; noting changing conditionsand needs; re-evaluating care objectives; modifying nursing careplan. Supervises medical record assuring accuracy, completeness andcompliance with State and Federal regulations, legal and ethicalstandards. Participates in QI activities. Develops expertise inMedicare, MDS assessment, Case Mix, payer sources and reimbursementissues. Maintains financial integrity of the unit via accuratepatient charges. Reviews, updates and standardizes forms. Maintainscurrent procedure manual and policies. Serves as a resource personto other healthcare professionals, nursing students and communitymembers. Responsibilities include interviewing, hiring, developing,training, and retaining employees; planning, assigning, and leadingwork; appraising performance; rewarding and coaching employees;addressing complaints and resolving problems.
