Our Kathleen, GA CrisisSupport Program is looking to add a Licensed Practical Nurse ( LPN) House Manager to the team! The LPN House Manager will serve as amember of the Intellectual and Developmental Disabilities (IDD)Crisis Support Home team and serve as the home manager who isresponsible for staff supervision, health and safety of individualsin the Crisis Support Home and coordinate and provide services tothose receiving Crisis Support Home services. The focus of thisprogram is to provide time-limited crisis services that supportindividuals with developmental disabilities in the community. TheIDD Crisis Home is used as a measure of last resort for individualsundergoing an acute crisis that presents a substantial risk ofimminent harm to self or others. The goal is to stabilize theindividual through nursing and behavioral supports, on a timelimited basis.

Advancement Opportunities Job Responsibilities:

Provide direct monitoring, assessment, and staff training of healthcare services including, specific interventions, treatmentprotocols, improvement of health care outcomes, etc. Supervise andschedules direct support staff and CNAs. Function as a liaison andadvocate for the individuals when dealing with physicians,psychiatrists, hospitals, and consultants including RD, OT, PT,SLP, etc. Provide on-going health care training and monitoring ofBenchmark staff as required by state and federal regulations, andBenchmark Crisis Assistance policies and procedures. Uses positivebehavior support strategies as described in behavior support plansor behavior guidelines. Responsible for maintaining priorauthorizations or obtaining medications and treatments covered byclient’s insurance plan or approved contractors. Responsible forscheduling and responding to quarterly pharmacy reviews. Completedaily, weekly, monthly, quarterly and yearly paperwork andassessments of clients’ health care needs Provide guidance duringclient emergency medical situations. Monitor and maintain clients’medical file to ensure that all documentation is current.

Facilitate communication between all medical services providers,IDT, LSCW, and supervisor to ensure quality of care. Attend allagency, departmental, and client specific meetings, includingclient appointments as directed. HR Qualifications:

Valid unrestrictedGeorgia Nursing License to practice nursing in the state of GAunder the clinical supervision of a Registered Nurse. Certificationand ongoing training in crisis intervention curriculum. Valid CPRand First Aid Certification 2 years experience working withindividuals with intellectual and/or developmental disabilitiesand/or mental illness who are in crisis. Valid Driver’s License andauto insurance. Minimum of year supervisory experience Must becomputer literate.
