JobPurpose:

The Director of Nursing, under the supervision of theAdministrator, provides clinical oversight over all patient careservices and staff providing care treatment and services topatients of the agency on a daily basis.

Key Responsibilities:

• Implement employee relations practices necessary to establishpositive partner-partner relationships and promote a high level ofpartner morale.
• Provide oversight coordinating referrals to theagency with internal and external referral sources.
• Screenpatient referrals for appropriateness for admission.
• Evaluate theagency’s ability to meet the anticipated clinical needs of patientsprior to accepting the admission.
• Provide oversight related tomaking patient and partner assignments.
• Assure the development,implementation, and updates of the patient’s individualized plan ofcare.
• Provide oversight regarding all aspects of coordination ofpatient care delivery.
• Assure that patient needs are continuallyassessed.
• Honor patients/residents’ rights to fair and equitabletreatment, self-determination, individuality, privacy, property andcivil rights, including the right to wage complaints.
• Participatein center/agency surveys (Licensure / JCAHO) and any subsequentlyrequired reports.
• Maintain operations daily workflow by timelycompletion of tasks assigned to this role within the Home Care HomeBased (HCHB) system.
• Utilize reporting capability for keyindicators within HCHB and Strategic Healthcare Partner to achievedesired outcomes for patients, billing and financial success.
• Collaborate with other key partners within organization to achieveagency clinical operational and financial success.
• Coach, mentorand serve as a preceptor to clinical partners to achieve positivemorale and retention objectives.

Qualifications Minimum Education Required:

• Nursing Degree from anaccredited college or university or is a graduate of anaccredited/approved RN program.

Minimum Experience Required:

• Two(2) years recent experience as an RN.
• One (1) year experience inhome health required.

Minimum Licensure/Certification Required by

Law:

• Current and unrestricted Registered Nurse (RN) licensure instate of practice.
• Current CPR certification.

AdditionalQualifications:

(Preferred qualifications)

• Bachelor of Science inNursing (BSN) preferred
