Responsibilities:
Acts as a liaison between hospice and the facility to maintain effective communication Upholds a positive image to the public through effective planning, programming, advertising, special events, and care management Specializes in professional Nursing care to patients in facilities Interacts with patients, families, physicians, and other Health team members Reviews admission data as defined by policies and procedures Assesses patient status and responds in timely manner Implements priorities for Nursing action in logical sequence according to patient’s needs Formulates a plan of care by the appropriate selection of Nurse Practice standards Supervises the provision of services provided by LPNs based on the plan of care and provides feedback on performance as indicated
Qualifications:
Associate degree in Nursing with current license One year practice as a Registered Nurse Acute Care, Home Health, or Hospice experience preferred Knowledge of Hospice Philosophy of Care About our
