We seek Motivating professionals who will join our Committed team. The VNA continues to grow and we are a seeking a PRN Care Management RN dedicated to delivering the highest quality in-home care management. This position has flexible hours, including day, evening, weekend or a combination! As a RN Care Manager you will direct care to clients and their families using a multidisciplinary approach. This will include conducting start of care admissions and assessments to identify problems and necessary services. Develop a professional care plan from the assessment findings and in collaboration with the clients Physician. Ensure in-home care delivery is in accordance with each client’s contractual program. Communicate explanation of the care plan and all subsequent cost of all services to client and or Families, Attorney, Trust Officers or Guardians. Must be available up to 15 hours per week with possibility to grow into a full time position. Required Qualifications Registered Nurse with 1 year of experience of in home case/care management Experience working with the adult population Knowledge of community health, social service agencies and additional community resources Ability to travel to client’s residence within 30 to 40 miles Excellent communication and interpersonal skills with the ability to quickly build rapport Ability to work with minimal supervision within the role and scope Ability to use a variety of electronic information applications/software programs including electronic medical records Valid driver’s license, car insurance, and access to an automobile Licensed to work in the State of Florida – will need ACHA Level II Background clearance Preferred Qualifications BSN Bilingual ‘ English, Spanish Experience with Private Pay Home Healthcare Why join our VNA family?
