The intake coordinator acts as liaison to case management ensuring patient care.

What will you come with:

Minimum of some college coursework Minimum of one (1) year of nursing experience, in home healthcare preferred Active Practical Nurse (LPN) license in the State of Florida. Specialized knowledge Knowledge of the Home Health Services Knowledge of referral/authorization process Knowledge of computer clinical programs such EHR/EMR system(s) Skills & Abilities Great communication skills, both written and spoken. Bi-lingual preferred Strong organization skills and be detail oriented Ability to maintain confidentiality of sensitive information.
