- Hybrid work Responsibilities Role Description We are looking for a Care Manager who will work in a clinic setting, focusing on monitoring patients with chronic illnesses and ensuring they follow prescribed care plans.

- Coordinate chronic care plans for patients in CCM and RPM programs.
- Track patient data from remote monitoring devices to identify trends.
- Check in with patients (by phone or in-person at the clinic) to assess progress.
- Communicate important health updates to physicians and clinical teams.
- Educate patients about medication, lifestyle, and self-management techniques.
- Document patient interactions and clinical updates in the EHR system.
- Schedule and oversee follow-up appointments and referrals.
- Help patients navigate available healthcare services and resources.
- Escalate any potential risks or health changes to providers promptly.
- Foster ongoing patient engagement to boost long-term health outcomes. Qualifications
- Current Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) certification.
- Familiarity with Chronic Disease Management and Remote Patient Monitoring.
- Proficiency in using EHR systems for documentation and care coordination.
