Savannah, GA $10,000 associated sign-on bonus This is not a remote or work-from-home position. This positionrequires you to sit on-site at our Savannah, GA branchlocation. The RN Clinical Manager coordinates and oversees all direct carepatient services provided by clinical personnel. This role is afocused on both home health clinical quality assurance and homehealth clinical operations initiatives. Develops, plans, implements, analyzes, and organizes clinicaloperations for a specific location managed. Conducts/delegates the assessment and reassessment of patients,including updating of care plans and interpreting patient needs,while adhering to Company, physician, and/or health facilityprocedures/policies. Manages the assignment of caregivers. Responsible for and oversees the delivery of care to allpatients served by the location. Receives case referrals. Reviewsavailable patient information related to the case, includingdisciplines required, to determine home health or hospice needs.
Accountable to ensure patients meet admission criteria and make thedecision to admit patients to service. Assigns appropriateclinicians to a case, as needed. Instructs and guides clinicians to promote more effectiveperformance and delivery of quality home care services and isavailable at all times during operating hours to assist cliniciansas appropriate. Assists clinicians in establishing immediate and long-termtherapeutic goals, in setting priorities, and in developing patientPlan of Care (POC). Monitors cases to ensure documentation is following compliancewith regulatory agencies and requirements of third-party payers.
Ensures final audits/billing are completed timely and in compliancewith Medicare regulations. Coordinates communication between team members/attendingphysicians/caregivers to ensure the appropriateness of care andoutcome planning. Works in conjunction with the Branch Director and CompanyFinance Department to establish location’s revenue and budgetgoals. Participates in sales and marketing initiatives. Supervises all clinical employees assigned to a specificlocation. Responsible for the overall direction, coordination, andevaluation of the location. Carries out supervisoryresponsibilities in accordance with Company policies andprocedures. Handles necessary employee corrective action and disciplineissues fairly and objectively, in consultation with the HumanResources Department and the Executive Director/Director ofOperations. Participates in the interviewing, hiring, training, anddevelopment of direct care clinicians. Evaluates their performancerelative to job goals and requirements. Coaches staff andrecommends in-service education programs, when needed. Ensuresadherence to internal policies and standards. Assesses staff education needs based on own the review ofclinical documentation in addition to feedback and recommendationsby Utilization Review staff. Upon completion of the assessment,creates and conducts regular staff education as needed. Analyzes situations, identifies problems, identifies andevaluates alternative courses of action through the utilization ofPerformance Improvement principles. Responsible for review of the appropriate number of CaseManagers and clinical staff documentation to includestarts-of-care, resumption-of-cares, and re-certifications, forappropriateness of care, delivery, and documentationrequirements. Responsible for the QA/PI activities. Works with UtilizationReview staff relative to data tracking for performance review andoutcomes of care analysis to determine efficiency, the efficacy ofcase management system as well as any other systems and process.
Competently performs patient care assignments and staff managementactivities. Provides direct patient care on an infrequent basis and only intimes of emergency. Acts as Branch Director in his/her absence. Interprets Company standards and Company policies andprocedures to ensure compliance with external regulatoryauthorities and ensures that caregiver clinical documentation meetsinternal standards. Required Experience/Skills:
Graduate of an accredited School of Nursing. Current state license as a Registered Nurse. Proof of current CPR. Valid driver’s license, auto insurance and reliabletransportation. Two years as a Registered Nurse with at least one-year ofmanagement experience in a home care, hospice or equivalentenvironment. Home health experience is required. Management and people leadership experience, required. OASIS experience, required. OASIS certification (HCS-O, COQS,and/or COS-C), preferred. Homecare Homebase (HCHB) experience, preferred.

billing knowledge and/or experience, preferred.
