MDS Lead Tower Road Post Acute 26 Tower Rd NE, Marietta, GA 30060 About Us Tower Road Post Acute is a leading skilled nursing facility dedicated to providing exceptional, compassionate care to our residents. We are seeking an experienced and detail-oriented MDS Lead to join our interdisciplinary team and ensure the highest standards of care and compliance. Job Summary The MDS Lead will oversee the coordination and completion of the Minimum Data Set (MDS) assessments for all residents in our skilled nursing facility. This role is critical to ensuring accurate resident assessments, care planning, and compliance with federal and state regulations, including CMS guidelines. The MDS Lead will collaborate with nursing, therapy, and administrative staff to optimize resident outcomes and reimbursement processes.

Key Responsibilities MDS Coordination:

Oversee the timely and accurate completion of MDS 3.0 assessments, including scheduling, data collection, and submission in compliance with CMS and state regulations.

Care Planning:

Lead interdisciplinary team meetings to develop and update individualized care plans based on MDS assessments and resident needs.

Compliance:

Ensure adherence to all federal and state regulations, including RAI (Resident Assessment Instrument) guidelines and Medicare/Medicaid requirements.

Quality Assurance:

Monitor and audit MDS documentation for accuracy, completeness, and compliance, implementing corrective actions as needed.

Training & Leadership:

Train and mentor nursing staff on MDS processes, documentation standards, and regulatory requirements. Provide leadership and guidance to the MDS team.

Reimbursement Optimization:

Collaborate with billing and administrative teams to maximize reimbursement through accurate RUG (Resource Utilization Group) scores and documentation.

Data Analysis:

Analyze MDS data to identify trends, improve care delivery, and support quality improvement initiatives.

Interdisciplinary Collaboration:

Work closely with nursing, therapy, social services, and other departments to ensure comprehensive resident care and accurate documentation.

Regulatory Updates:

Stay current on changes to MDS, CMS, and other regulatory requirements, implementing updates to processes as needed.

Qualifications Education:

Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid state license.

Experience:

Minimum of 2-3 years of MDS experience in a skilled nursing facility, with at least 1 year in a leadership or supervisory role.

Certifications:

RAC-CT (Resident Assessment Coordinator-Certified) preferred or willingness to obtain within [insert timeframe, e.g., 6 months].

Skills:

Strong knowledge of

MDS 3.0, RAI

process, and CMS regulations. Excellent organizational and time-management skills. Proficiency in electronic health records (EHR) systems and MDS software (e.g., PointClickCare, MatrixCare). Strong leadership, communication, and interpersonal skills. Ability to analyze data and implement quality improvement initiatives.
