Difference :
The Director of Care Management (RN) is responsible for managing the day-to-day oversight and adherence to all standards and expectations of the Clinical and Social Services team. They will report directly to the VP of Clinical Services and will collaborate with the executive team to improve processes, drive performance management and ensure operational quality. The Director of Care Management (RN) will maintain and ensure successful execution and performance management of all care programs as required by Arkos Health and its contractual requirements.
Essential Duties and Responsibilities:
Execute, and ensure performance of, all clinical and social services programs established by Arkos Health in accordance with our contractual requirements Provide supervision, guidance, and ongoing training leadership to the case/care management and social services team. Conduct performance evaluations and formulate disciplinary action plans, as appropriate. Collaborate with the Arkos Health leadership to ensure care management compliance according to corporate and compliance policies Work collaboratively with Director of Quality and other staff to ensure HEDIS and STAR measure performance. Prepare and appropriately deliver reports and agendas as required and participates in meetings according to contracted standards Conduct weekly audits of department cases, to ensure corporate and partner compliance. Review audits with Clinical Services Auditor. Prepare and investigate quality issues or complaints involving the department Participate in after-hour and week-end Administrator on call coverage Proactively interact with network providers and offices to educate on organization services Perform other duties as assigned to support the overall effectiveness of the organization
Qualifications and Skills:
Bachelor’s degree in Nursing (BSN) and current and valid State of Georgia RN Licensure Five years of experience leading ambulatory or community (in-home) care / case management programs, including social services Knowledge of CMS managed care, Medicare/Medicaid state, and local managed care regulations Ability to present clinical information accurately and confidently Able to communicate effectively (orally and in writing) and professionally with all levels of internal staff and external customers Current unrestricted state driver’s license with personal reliable transportation and proof of liability insurance coverage Current BLS/CPR certification, TB test and maintain a negative test annually required Hep B vax/titer proof (Company does offer/or may sign a declination) required Current flu shot (or sign waiver/follow mask policy) required
Preferred Qualifications & Skills:
