This RN professionalcan apply the Synergy Model for Care in practice, and use thisknowledge to personalize care for each patient/family while servingas a resource to colleagues and strong advocate for them.

Thisindividual will:

Serves as a resource to colleagues and strong advocate forpatients/families. Consistently communicates effectively andrespectfully with the patient, the patient’s family, and othermembers of the healthcare team. Delivers safe, dependable andeffective care to a patient population independently utilizing thenursing process. Demonstrates developed assessment skills.

Recognizes the impact of nursing care on patient outcomes. Continues to recognize own performance limitations, butrequires minimal assistance. Documents the nursing processaccurately and in the appropriate format (e.g. computer, clinicalpathway, chart). Plans and implements patient care independently.

Utilizes patient/family input and goals in planning plan of care.

Evaluates patient’s response to planned care and adjusts plan ofcare as appropriate. Identifies patient and family learning needsand makes recommendations to resources regarding those needs. Functions as Charge Nurse when assigned. Orients staff orstudents as assigned in absence of designated preceptor. Utilizesresources in a responsible manner with a focus on costcontainment. Identifies and implements a course of action in non-routinesituations. May seek assistance to resolve complex issues. Respectsand maintains confidential information. Participates within the unit to identify and address disciplinespecific issues and needs. Articulates application of Synergy modelfor care in clinical practice. Collects, reports, analyzes and provides data on an ongoingbasis and as required by physician and/or interdisciplinary teamsin accordance with organization standards. Revises and evaluates effectiveness of patient caredocumentation. Uses interviews, surveys, focus groups andquestionnaires to identify and resolves areas for improvement.

Meets all Clinical Nurse I Employee Commitments. Achieves pointsand contact hours as defined in the PLAN (clinical lattice) pointsystem to maintain position.

Minimum Qualifications Graduate of an accredited school of nursing. 12 months ofrecent nursing experience within the past five years or recentcompletion of a re-entry nursing program. Must have a valid, active unencumbered Nursing license ortemporary permit approved by the Georgia Licensing Board. BLScertification required and ACLS may be required in designateddepartments.
