Growing together. The Clinical Team Manager is an RN position who performs managementfunctions in coordination of all clinical disciplines including:skilled nursing, home health aide, physical therapy, occupationaltherapy, speech therapy, and medical social services.

  • The ClinicalTeam Manager coordinates patient care on all activities and assumesresponsibility for continuity, appropriateness and quality ofservices delivered within the home health department.
  • The ClinicalTeam Manager assists with the day-to-day scheduling of staff,monitors performance of staff, assists in the orientation of staffand serves on committees to improve operations.
  • The Clinical TeamManager performs all functions in a professional and ethical mannerand collaborates with other members of the health care team toensure the provision of quality care.

The Clinical Team Manager isresponsible for verification of appropriate incoming ordersobtaining physician orders and completion of orders. Receive andrespond to all incoming calls from referral sources or potentialpatients, exchange information to identify patients’ needs, anddetermine the Company’s ability to meet them. The Clinical TeamManager is responsible for communication with the schedulingcoordinator in reference to new physician orders and assuring thepatient is staffed appropriately along with the reviewing ofunassigned patient visits and assisting with the staffing of thepatient visits with the appropriate field staff member. Responsiblefor performing on call for home health department approximately oneweek per month. Clinical Team Manager will work under thesupervision of the Clinical Team Manager Lead per branch and/orDirector of Nursing for home health for that branch.•

Primary Responsibilities:

Provides direction and leadership to designated Agencypersonnel in an effort to ensure quality and continuity of servicesthrough competent personnel management skills Ensures that all supervised health care staff maintainscompliance of federal and state rules and regulations and to agencyclinical policies, procedures, and appropriate standards ofcare Ensures that a client’s plan of care is executed as written andper physician orders Ensures that a reassessment of a client’s needs is performed bythe appropriate health care professional When there is a significant health status change in theclient’s condition; at the physician’s request; or after hospitaldischarge Participates and assists in orientation and training ofclinical and paraprofessional staff Ensures that on-site supervisory requirements of designatedstaff are met on a timely basis Demonstrates an effective leadership style and solves problemsin an efficient and logical manner after accurately analyzingfacts Ensures scheduling and assignments are appropriately based onpatient needs, staff expertise, and geographical locations Assists in the coordination of patient services, to include labreports, updated patient reports, and interdisciplinarycommunication Receives and coordinates patient referrals from case managers,physicians, and other referral sources Participates in interdisciplinary care conferences, Agencyprograms and management functions, as assigned Monitors documentation and paper flow to ensure records areaccurate and timely and adhere to Medicare and other payerguidelines Ensures that clinical notes, supply log/visit logs are accurateand turned into the office according to Agency policy Provides assistance to CTM with investigation of patient/familycomplaints promptly and takes corrective action as needed Makes home visits to provide patient care as needed Recognizes and reports life threatening situations and respondsappropriately Demonstrates knowledge of emergency procedures and respondsappropriately Participates in the Agency’s endeavors for accreditation,licensing and professional recognition according to state, federalor CHAPs requirements Assists CTM in the development of Performance Improvement Planreports to include achieved goals, opportunities for improvement,statistical data and projections Participates in home care Performance Improvement Programactivities to continuously improve the quality of patientcare Serves as a resource person for clinical staff and managementin matters pertaining to Performance Improvement Participates in periodic review of clinical records to promotequality patient care Educates clinical staff on documentation standards and patientcare as needed Monitors assigned cases to ensure compliance with requirementsof third party payers Attends all required in-service programs to maintaincomprehensive home care knowledge base within the review year, asassigned by supervisor Demonstrates effective time management skills by completingassignments within projected time frames Expresses verbal and written communication in a clear, positiveand cooperative manner Demonstrates personal responsibility with regard to attendanceand punctuality Promotes Agency philosophy, mission statement andadministrative policies to ensure quality of care Maintains privacy and confidentiality with regard to allpatient, staff and Agency information Provides positive, supportive communication to patients,families, visitors, physicians and other personnel Promotes the Agency image by adhering to the DressCode Meets productivity requirements based on Agencystandard Demonstrates knowledge and observance of the Patient’s Bill ofRights Follows all infection control, standard precautions, and safetyguidelines/standards per Agency policy Follows the appropriate chain of command in all forms ofcommunication Demonstrates flexibility, enthusiasm and willingness tocooperate while working with others or in place of others asnecessary Must be flexible with work schedule, able to work weekdays-oncall and weekends per assigned schedule Performs other duties as assigned In 2011, WellMed partnered with Optum to provide care to patientsacross Texas and Florida. RequiredQualifications:

Graduate of an accredited school of nursing Current licensure as a Registered Licensed Nurse in good standingwith the Board of Nurse Examiners for the State of Texas Certification in Basic Cardiac Life Support Course C. Familiaritywith medical terminology and equipment.

  • Additional educationand/or experience may be required by individual stateregulations 3+ years of experience as a RN in an acute care setting or generalmedical/surgical setting Evidence of independent nursing practice in delivering nursingcare Proven basic knowledge of governmental regulations, Medicare(Medicaid, as required), regulations, and company policies andprocedures
  • Proven good organization, communication, human relations skills Demonstrated self-motivation, assertiveness and independentdecision-making skills
  • Ability to read and follow written and verbal instructions inEnglish effectively Possesses ability to operate computer terminal, printer, fax andcopier machines Driver’s License and access to reliable transportation This position requires Tuberculosis screening as well as proof ofimmunity to Measles, Mumps, Rubella, Varicella, Tetanus,Diphtheria, and Pertussis through lab confirmation of immunity,documented evidence of vaccination, or a doctor’s diagnosis ofdisease•

  • The salary range for this role is $71,600 to $140,600 annuallybased on full-time employment.
