Responsible for assessment, care coordination,discharge planning, and post-acute referrals for patients withcomplex medical and/or social determinants of health needs in theacute hospital setting.

Job Essentials:

Coordinates the care andservices for patients identified as needing assistance or meetingCare Management criteria.

Collects in-depth information about apatients medical, functional, and social condition, to identifyindividual needs in order to develop a plan to meet thoseneeds.

Works with patients and family/caregivers to determinespecific goals and actions based on assessment.

Coordinatesdischarge planning.

Executes specific interventions to meetestablished goals.

Organizes, integrates and coordinates thenecessary resources to accomplish the goals and plan.

Assists thecare team in developing a plan of care which includes but is notlimited to assuring appropriateness of services and care setting,assuring individualized support and education, determining the needfor continued services, planning for discharge, and identifying andconnecting patients/families with available community resources ifneeded.

Collaborates with Physicians and other members of the healthcare team on the patients behalf.

Maintains up-to-date and accuratedocumentation of patient assessment and plan of care to ensureeffective integration of information for use by the health careteam to ensure on-going and continued quality of care.

Identifiesappropriate admission and continued stay issues.

Enhances thequality of patient care through effective and efficient use ofresources.

Helps to identify strategies for reducing length of stayand appropriate utilization of services.

Utilizes criteria includingclinical pathway data and implements strategies to resolvecontrollable variances.

Attends, facilitates and participates ininterdisciplinary rounds and case conferences.

Advocates for patientrights.

Promotes mission, vision, and values of IntermountainHealth, and abides by service behavior standards.

May be required towork days, evenings, weekends, and holidays.

Performs other dutiesas assigned.

Minimum QualificationsGraduation from an accreditedSchool of Nursing is required.

BSN or ASN/ADN degree with BSN degreerequired within 4 years of hire.3 years Registered Nurseexperience.

Preferred:

BSN at time of hire is preferred.3 years CareManager experience in an acute or ambulatory setting ispreferred.

Physical Requirements:

Ongoing need for employee to seeand read information, documents, monitors, identify equipment andsupplies, and be able to assess customer needs. Frequentinteractions with providers, colleagues, customers,patients/clients, and visitors that require employee to verballycommunicate as well as hear and understand spoken information,needs, and issues quickly and accurately. Manual dexterity of handsand fingers to manipulate complex and delicate equipment withprecision and accuracy. This includes frequent computer use fortyping, accessing needed information, etc.
