The team is fun, flexible, and diverse, working together toward a common goal and sharing our strengths and perspectives. The Nurse Care Manager serves as a member of the BH CP team, providing direct care management and coordination to Enrollees. The Nurse Care Manager collaborates with medical and other care providers and provides consultation and guidance to team members regarding health needs, health behaviors and medications of the team’s Enrollees; as well as participates in the ongoing development of trainings for staff and Enrollees regarding health and health behaviors. This position is 20 hours per week; Monday-Friday during daytime hours doing work in the office, some in the community seeing members, and some remote work flexibility. Bilingual Spanish required for this position with a $2,500 language differential. What you’ll do: Provide all nursing and related aspects of care management and coordination services to the Enrollees of the BH CP, including but not limited to medication reconciliation, discharge planning, coordination with physicians, reviewing Enrollee Assessment health information, collaborating on Assessments and Care Planning, etc. Consult with BH CP staff, Program Director, Medical Director, and Care Team members, PCP and/or MCO/ACO regarding Enrollees’ health related activities, interventions and service needs, as needed. Communicate directly with Enrollees via telephonic, electronic and face to face modalities Collaborate with program management and staff to develop and provide training for staff and Enrollees on health and wellness, and healthy behaviors. Provide other direct services in keeping with the Nurse Care Manager role, including serving as primary care coordinator for some Enrollees, as may be assigned by the Clinical Care Manager/Team Leader. Complete all clinical and administrative documentation in a thorough, efficient and timely way, in compliance with organization, payer, state and other applicable protocols. Comply with all mandated reporting requirements and regulations (i.e. DCF, DMH, DPPC, Elder Protective Services). What you’ll need:
Education/Certification:
Registered Nurse with current Massachusetts license to practice. Bachelor’s degree in nursing preferred.
Experience:
Experience with people with mental illness and substance use disorders, with at least one year of providing care to individuals with medical problems preferred. Embraces person-centered planning philosophy and motivational interviewing Other requirements: Valid Mass Drivers License and access to reliable vehicle required. Bilingual English/Spanish required.
