Job Description
Exciting opportunity to join a fast-paced medical team within a growing federally qualifiedmunity health center. HealthFirst Family Care Center, Inc. “The Heart of Our Community” providesprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care.
RN Case Manager/CDE Summary:
Care Management is theponent that links patients with broader healthcare and social support systems that ensure timely follow-up. Under the general supervision of the Clinical Director, the RN Case Manager performs the primary role functions of assessment, planning, facilitation and advocacy that is achieved through collaboration with the patient’s care team.
Schedule:
Mon-Fri 8:00 am – 4:30 pm
Required Qualifications:
1.License to practice as an RN from the Commonwealth of Massachusetts and BA/BS degree or higher in health and human services.
2.Certified Diabetes Educator (CDE) or Certified Diabetes Outpatient Educator (CDOE) required to obtain within 90 days of hire.
3.Minimum two years’ RN experience with direct nursing care ormunity setting.
4.RN Case Manager experience along the care continuum addressingprehensive care, involving all parties, and including client contact.
5.Kledge of health, social service and funding sources.
6.Appropriate continuing education.
7.Maintain current certification in CPR at the healthcare provider level.
8.Strongputer kledge.
9.Bi-lingual skills preferred.
10.Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.

1.RN Case Manager must project a positive attitude about HealthFirst Family Care Center, Inc.
2.Strongputer skills required toplete online applications.
3.Must possess excellent oral and writtenmunication skills to effectivelymunicate with patients and other agencies.
4.Ability to handle confidential information in a professional manner and in accordance with HIPAA guidelines.
