Schedule:
Monday through Friday (No Weekends/Holidays)
Role Overview:
Coordinate long-term care needs for patients in the local community.
Relay member needs and advocate for optimal care.
Work remotely if within a commutable distance of the Boston Area.
Primary Responsibilities:
Assess, plan, and implement individualized care strategies.
Initiate referrals for social service programs.
Manage care plans as a single point of contact.
Communicate health-related information to ensure coordinated care.
Advocate for patients and families.
Qualifications:
Current RN licensure in Massachusetts.
2+ years of clinical experience.
1+ years of experience with MS Office.
Ability to travel within the assigned region.
Preferred Qualifications:
BSN, Master’s Degree, or higher in Clinical Field.
CCM Certification.
1+ years of community case management experience.
Experience in team-based care and managed care.
