Holy Trinity Nursing & Rehab Center, a leading skilled nursing facility in Worcester, MA, is seeking a dedicated and compassionate Assistant Unit Manager Nurse (LPN or RN) for our busy 37-bed short-term rehabilitation unit. This unique role combines direct patient care leadership with essential case management responsibilities, offering a challenging and rewarding opportunity to make a significant impact on the recovery journeys of our residents.
About Our Unit:
Our 37-bed short-term rehabilitation unit provides intensive, goal-oriented therapy and nursing care to residents recovering from acute illnesses, injuries, and surgeries, preparing them for a safe return home or to a lower level of care. It’s a fast-paced environment that demands excellent clinical skills, strong organizational abilities, and a true passion for rehabilitation.
Key Responsibilities:
Assistant Unit Manager Duties (approx. 60%): Assists the Unit Manager in the day-to-day operations of the 37-bed short-term rehabilitation unit. Provides direct supervision and mentorship to nursing staff (LPNs and CNAs), ensuring high-quality patient care and adherence to all policies and procedures. Participates in resident admissions, transfers, and discharges, ensuring seamless transitions of care. Conducts comprehensive nursing assessments and develops individualized care plans. Administers medications and treatments as prescribed, ensuring accuracy and safety. Monitors and evaluates resident progress, reporting changes in condition to the Unit Manager and physician. Collaborates with physicians, therapists, and other healthcare professionals to optimize resident outcomes. Assists with scheduling, staffing, and performance evaluations as directed by the Unit Manager. Ensures compliance with all federal, state, and local regulations, as well as facility policies. Participates in quality improvement initiatives and resident care conferences. Acts as a clinical resource and role model for nursing staff. Case Manager Duties (approx. 40%): Serves as the primary case manager for a designated caseload of short-term rehabilitation residents. Coordinates and facilitates interdisciplinary team meetings to discuss resident progress, discharge planning, and any barriers to recovery. Develops and implements comprehensive discharge plans in collaboration with residents, families, physicians, therapists, and community resources. Identifies and addresses resident and family needs, providing education and support throughout the rehabilitation process. Advocates for residents’ needs and ensures access to appropriate services and resources. Maintains accurate and thorough documentation of all case management activities. Collaborates with insurance companies and other payers to ensure appropriate authorization and reimbursement for services. Facilitates communication between residents, families, and the healthcare team.
Qualifications:
Current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse license in the Commonwealth of Massachusetts. Bachelor of Science in Nursing (BSN) preferred. Minimum of 2-3 years of recent clinical experience as an LPN/ RN in a skilled nursing facility, acute care, or rehabilitation setting, with a strong emphasis on short-term rehab. Previous experience in a leadership role (e.g., Charge Nurse, Team Leader) is highly desirable. Demonstrated experience or strong interest in case management or discharge planning is a plus. Excellent assessment, critical thinking, and problem-solving skills. Strong interpersonal, communication (verbal and written), and organizational skills. Ability to work effectively in a fast-paced, demanding environment. Proficiency in electronic health records (EHR) systems. Compassionate, patient-centered approach to care.
