Nursing Case Manager Starting salary is $70,000 per year, with flexible scheduling options, manageable patient ratios, on-demand pay, mileage reimbursement, and numerous other benefits! If you have a nursing background combined with case management experience, and if you are passionate about creating care plans that genuinely enhance individuals’ lives, then consider the Nursing Case Manager position. In this role, you will be responsible for developing and implementing comprehensive care plans for home care services, ensuring that all services provided meet the highest quality standards, as well as adhering to licensing regulations and our company’s protocols. Conduct intake assessments for the individuals we serve, ensuring timely submission of necessary documentation to secure prior authorizations for services based on each individual’s payment sources. Create individualized plans of care driven by nursing diagnoses alongside input from the individual and their physician, which encompasses medical interventions and clearly defined measurable goals. Regularly evaluate relevant medical data to assess the success of the provided services in achieving the highest level of rehabilitation. Address any concerns or complaints from individuals or employees, taking appropriate actions in response. Oversee the orientation and training of aides to guarantee they are fully equipped to meet each individual’s needs according to agency standards. Conduct periodic supervisory visits for aides, providing written evaluations on the delivery of services. Educate individuals, family members, and primary caregivers as needed to ensure optimal functioning levels are maintained or achieved. Facilitate the effective coordination of home care services by ensuring documentation is promptly completed and relevant medical data is transferred to physicians, therapists, or other caregivers without delay. Contribute insights toward the development or modification of agency policies and procedures to enhance service delivery. Engage with advisory boards or quality assurance committees as requested to support agency initiatives.
Qualifications:
A Bachelor’s Degree in Nursing and a valid state nursing license. Minimum of two years of relevant experience in nursing case management. A self-starter with strong attention to detail and the capacity to handle multiple tasks effectively. Exceptional communication and analytical abilities.
