The Clinical Team Leader is a Registered Nurse responsible for assisting the Executive Director/Administrator with day-to-day patient care management and contributes to the overall success of the company. Responsibilities Effectively facilitate the relationship between physicians, referral sources, caregivers, employees, and patients. Direct daily Stand Up Call. Assist the Executive Director/Administrator and Intake/Scheduler with daily scheduling process to ensure timely admissions and visits are made in a compliant and efficient manner. Coordinate admissions with sales/clinical liaisons and RN’s. Coordinate admissions and discharges with field staff and facilities. Provide supervision of patient care and review/approve orders. Oversight of
HIS/CAHPS
on the branch level. Oversight of recertification process; Face to face visits and CTI’s. Audit nursing, hospice aide, chaplain, and social worker documentation quarterly to ensure compliance and quality. Review wound documentation and pictures for compliance with policy. Oversee ordering, inventory and monthly reconciliation of all medical supplies, pharmacy and DME in the branch. Prepare and direct IDG. Ensure all staff updates are completed prior to IDG. Follow up on new/prior tasks from IDG meetings. Review operational/clinical reports, as delegated by ED/ADM, for outliers and act on opportunities. Assist the Executive Director/Administrator to provide ongoing education and training to all branch clinicians to ensure the documentation requirements are compliant with regulatory standards. Assist the Administrator in Survey process during preparation and survey visits. In collaboration with branch leadership participate in QAPI and Annual Agency Review. Must adhere to all Mayfair Management policies and procedures. Ensures confidentiality of patients and staff in accordance with HIPAA standards. Perform other duties as assigned. Job Qualifications:
Must hold a current and unencumbered Registered Nursing License in the state which you intend to work. Supervisory experience is preferred
Education:
Graduate of an accredited
Diploma, Associate or Baccalaureate School of Nursing License/Certification:
Current Driver’s License Current Registered Nursing License Reliable transportation and valid auto liability insurance
Experience :
2 years or more in a clinical setting, HH or Hospice preferred
Knowledge and Skills:
