Key Responsibilities:
Coordinate care for pediatric patients with complex medical, behavioral, or social needs Act as a liaison between families, providers, and external resources Monitor patient progress and follow up on treatment plans Provide education and support to families to help navigate the healthcare system Work collaboratively with clinical staff to ensure continuity of care Maintain accurate documentation in the electronic health record (EHR)
Qualifications:
Current state registered nurse license One years of experience in care coordination, case management, or pediatric healthcare Strong interpersonal and communication skills Experience with EHR systems and care management platforms Bilingual (Spanish/English) a plus Why Join Us?
