We are seeking a dedicated,compassionate, and experienced RN Supervisor for our Adult DayHealth program. This is a full-time, permanent position that offersa unique opportunity to lead a team of healthcare professionals inproviding exceptional care to our adult and geriatric patients. TheRN Supervisor will play a crucial role in ensuring that ourpatients receive the highest quality of care while also ensuringthat our program operates efficiently and effectively. Thisposition is ideally suited for a candidate with a strong backgroundin nursing, management, and geriatric care, as well as experiencewith the PACE (Program of All-Inclusive Care for the Elderly) modelof care.

Responsibilities:

Oversee the daily operations of theAdult Day Health program, ensuring that all patients receivehigh-quality care in a safe, respectful, and welcoming environment.

Lead, mentor, and supervise a team of healthcare professionals,including RNs, LPNs, CNAs, and other support staff. Develop andimplement care plans for patients, in collaboration with otherhealthcare providers, patients, and their families. Monitor thehealth status of patients, adjusting care plans as necessary andintervening in emergency situations. Ensure compliance with allrelevant regulations, standards, and best practices in healthcare.

Participate in the recruitment, hiring, training, and performanceevaluation of staff. Foster a culture of continuous improvement,encouraging staff to identify and implement best practices inpatient care. Communicate effectively with patients, families,staff, and other healthcare providers. Participate in thedevelopment and implementation of policies and procedures for theAdult Day Health program.

Qualifications:

Current and valid RNlicense. A minimum of 5 years of nursing experience, preferably ina geriatric care setting. At least 2 years of management orsupervisory experience in a healthcare setting. Experience with thePACE model of care is strongly preferred. Excellent clinicalskills, with a strong emphasis on geriatric care. Strong leadershipand management skills, with a proven ability to lead a team ofhealthcare professionals. Excellent communication and interpersonalskills, with the ability to communicate effectively with patients,families, staff, and other healthcare providers. Strongproblem-solving skills, with the ability to make sound decisions inemergency situations. A deep commitment to providing high-qualitycare to our patients. Proficient in using electronic health recordsand other healthcare IT systems. Ability to work a flexibleschedule, including some evenings and weekends. Must be physicallyable to perform the duties of the position, including lifting andmoving patients as necessary. This is a full time role,

Mondaythrough Friday:

40 hours a week with on call as needed.
