Role:

Join a collaborative and patient-centered care team as an RN Care Manager supporting Women’s & Children’s services at UNC Medical Center. This role focuses on care coordination, discharge planning, and support for complex patient needs. You’ll work closely with patients, families, and multidisciplinary teams to ensure safe, timely, and effective transitions in care.

Key Responsibilities:

Conduct assessments, develop care plans, and manage discharge processes using Epic™ Actively participate in CAPP and Complex Care meetings to address barriers and optimize progression of care Coordinate with social workers, utilization managers, and community resources Support patients and families with education, planning, and access to post-discharge services Monitor cases (20-25 at a time), document accurately, and maintain patient confidentiality Demonstrate professionalism, critical thinking, and a commitment to continuous improvement

Must Haves:

Current North Carolina RN license BLS certification 2+ years of acute care RN experience within the last 2 years (Pediatrics preferred) ASN accepted•Must enroll in BSN program within 4 years; complete within 7
