JOB SUMMARY
Assist with the process of admissions to include interviewing,assessing, and completion of all pertinent paperwork. Assess resident needs, develop, and implement a care plan foreach resident initially and on an ongoing basis as determined bythe needs of the resident. Report to and obtain orders from physicians regarding eachresident. May delegate this to Nurses, Medication Aides, Med Techsand Aides as appropriate and with supervision. Sets the tone and atmosphere for the Nursing Unit. Must be willing and able to work as a direct care nurse oraide. Must be willing and able to administer medications. Must be knowledgeable and adhere to all Liberty Senior LivingPolicies and Procedures and state rules and regulations. Supervisesservices for Facility in accordance with the Nurse Practice Act,Facility policies and procedures and the local, state and federalregulations. Obtain medication for each resident from contract pharmacy. Maydelegate this as appropriate and with supervision. Ensures treatments are completed by the Primary StaffNurse/Treatment Nurse as scheduled during shift and documentsappropriately as needed. Monitors specific skin tears and/or other skin conditions anddocuments at least weekly or more often if warranted in conjunctionwith treatment nurse (if appropriate). Performs CPR when needed. Deals with employee performance issues on a timely basis anddocuments actions. This includes issuing counseling forms andsuspending or terminating staff at the discretion of the Directorof Nursing. Makes suggestions toward improving patient care and/or workingconditions. Documents concerns and reports to Director of Nursing. Promotes and maintains the highest moral and professionalstandards Ensures labs and orders from consults are completed properlyand timely. Point Click Care – must utilize the program as designed andtrained. Must complete audits for Nurses, Medication Aides, MedTech and Aide task documentation. Must train and supervise theNurses, Medication Aides, Med Techs and Aides in the use of PointClick Care for each resident. Responsible for supervision the resident care employees andimplements and/or makes recommendation for disciplinary action asnecessary. Participates in preparing and delivering job performanceevaluations for resident care employees in accordance with companypolices and returns to the DON. Ensures there is proper documentation of all resident carestaff performance issues. Monitors, coaches and mentors to ensure ongoing qualityperformance of resident care staff. Assists in maintaining and updating job descriptions andemployees records. Orients, trains and leads by example to ensure resident carestaff are aware of and practicing by approved care policies andprocedures. Assists in training medication administration to appropriatestaff. Coordinate resident services with other professionals asordered by the physician or as assessment and care plandictate. Follow the facility infection control program, train staff,ensure compliance and assist with annual reviews. Order and maintain medical supplies as necessary with properaccounting program for charging residents for personalitems. Conduct, document, and report weekly, monthly, quarterly, andannual assessments of resident as prescribed by the Point ClickCare program. May delegate this to Nurses, Med Tech/Aides asappropriate with supervision. Coordinate with families and update families on a weekly and/ormonthly basis on the status of resident as well as contactingimmediately in the event of fall, sickness, etc. Maintain and supervise medication administration, medicationordering, medication count of controlled substances, and chartingfor each resident. Train resident care staff on emergency procedures to befollowed regarding fire, disaster, and resident incident and careissues. Ensure proper staff coverage. Assume and arrange for on-callresponsibilities and respond to calls of an emergency nature fromresident and/or supervisor on duty as needed even if reporting towork is necessary to assess the situation. Follow all policies and procedures, model relationship andleadership abilities, and work cooperatively and supportively withthe residents, family members and friends, professionals,Administrator, department heads, supervisors, and all otherstaff. Perform any other duties as assigned by the Director ofNursing. Must be willing and able to take on-call duty and be availableby telephone during on-call periods.
JOB REQUIREMENTS
Must be at least 21 and have a high school diploma orequivalent. Must be a Registered Nurse with 3 years’ supervisoryexperience in ACH, CRCF or Skilled Care. Must be qualified asSupervisor-in-Charge based on the ACH/Skilled rules of the State ofpractice. Must have experience in working with geriatric anddementia residents. Must have excellent communication andinterpersonal relationship skills. Must be able to be cheerful and represent a positive,professional first impression at all times. Must be willing to care for geriatric patients and have agenuine concern for their welfare. Must be willing to delegate, supervise, receive, understand,and follow orders. Must be willing to work and cooperate well with other nursingstaff, other departments, and families and visitors. Must be kind and respectful to patients, staff, andvisitors. Must be able to use equipment without jeopardizing the safetyof self, others, or equipment and use supplies safely andeconomically. Demonstrates neat appearance, dress code, and good personalhygiene. Must read, know, and follow personnel, department, andCommunity policies and procedures. Must attend in-service training and other staff meetings asrequired as well as complete online training.
