Job Summary:

nA Case Manager Registered Nurse (RN)coordinates and manages the care of patients, ensuring they receiveappropriate, cost-effective, and timely healthcare services. Casemanagers work across various healthcare settings, includinghospitals, outpatient facilities, and home care, to facilitatepatient care transitions, advocate for patients, and collaboratewith multidisciplinary teams to promote optimal outcomes.nn

KeyResponsibilities:

n n Conduct comprehensive assessments of patients’ medical,psychosocial, and financial needs to develop individualized careplans.n Coordinate and facilitate care across multiple healthcaresettings, ensuring continuity of care, appropriate utilization ofresources, and adherence to treatment plans.n Collaborate with physicians, healthcare providers, andinsurance companies to authorize treatments, secure necessaryservices, and ensure compliance with clinical guidelines.n Evaluate patient progress, monitor outcomes, and adjust careplans based on patients’ evolving needs and goals.n Educate patients, families, and caregivers on diseasemanagement, treatment options, and available resources to supportrecovery and self-care.n Advocate for patients to ensure they receive appropriateservices and that care is delivered efficiently andeffectively.n Facilitate communication among healthcare providers, patients,and insurance companies to resolve barriers to care.n Assist patients with obtaining necessary medical equipment,medications, and follow-up care to ensure successful transitionspost-discharge.n Track healthcare costs, utilization of services, and outcomes,contributing to cost containment strategies and improved caredelivery.n Monitor and ensure compliance with healthcare regulations,insurance policies, and external mandates related to casemanagement.n Participate in quality improvement initiatives, utilizingevidence-based practices to enhance patient care.n Provide guidance and support to patients and familiesnavigating complex healthcare systems.nnn

Qualifications:

Education:

Bachelor of Science in Nursing (BSN) preferred;Associate Degree in Nursing (ADN) accepted.n

Licensure:

n n Valid Registered Nurse (RN) license in the state ofpractice.nnn

Certification:

n n Case Management certification (CCM) preferred.n Basic Life Support (BLS) certification required.nnn

Experience:
