Role:
Morgan Stephens is seeking a Licensed Practical Nurse (LPN) Clinical Coordinator on behalf of a well-established home care organization based in New Hampshire. This is an exciting opportunity for a skilled LPN to join a dedicated leadership team focused on improving care coordination, supporting caregiver staff, and ensuring compliance and excellence in client services.
Key Responsibilities:
Clinical Oversight & Care Management Supervise and support LPNs and caregivers (HHAs/PCSPs) under the direction of the Clinical Director Determine service frequency based on patient acuity and needs Provide admission and direct care as needed Ensure proper staffing coverage Oversee individualized care plans and coordinate care with physicians and families Caseload & Clinical Coordination Manage caseloads of approximately 50 clients without primary nursing authorization Ensure timely completion of supervisory visits and recertifications Delegate clinical field visits to appropriate team members Intake & Documentation Review and input intake documentation and clinical data Maintain accurate records in the agency’s EMR system Support state and agency compliance standards Field Supervision & Compliance Monitor caregiver field supervision Track hospitalizations and changes in client condition Assist with resolution of clinical complaints and compliance issues Staff Support, Training & Development Provide field competency assessments and skills training Support onboarding for new clients and staff Conduct retraining as needed Collaborative Functions Assist with audits and performance improvement initiatives Participate in QAPI efforts, team meetings, and emergency planning
Qualifications:
Current Licensed Practical Nurse (LPN) certification in New Hampshire Minimum 5 years of clinical LPN experience, with 3+ years in a supervisory or management role preferred Home health experience required Strong interpersonal, documentation, and time management skills CPR/First Aid certification Reliable transportation and valid driver’s license

Ability to lift up to 50 lbs and perform patient transfers Must be able to walk, stand, kneel, and reach during patient care
Tools/Equipment Used:
Hoyer lifts, gait belts, walkers, glucometers, wheelchairs, etc.
